P21在乳腺癌中的表达及临床意义

摘　　要：目的通过检测P21蛋白在正常乳腺组织、乳腺导管内癌及浸润性乳腺癌中的表达，探讨其与乳腺癌临床特征、病理指标及复发转移和预后的关系。方法应用免疫组化SP法，检测22例正常乳腺组织，32例乳腺导管内癌，40例浸润性乳腺癌中P21的表达情况，结合临床病理参数进行统计学分析。结果浸润性乳腺癌、乳腺导管内癌、正常乳腺组织中P21的表达逐渐降低，浸润性乳腺癌与乳腺导管内癌及正常乳腺组织相比，差异具有统计学意义（P〈0．05）；乳腺癌组织P21的表达与PR呈正相关，与TNM（tumor node metastasis）分期、组织学分级、HER2的表达呈负相关（P〈0．05）；与脉管侵犯、肿瘤大小、患者年龄、月经状态及ER、Ki67的表达无相关性。结论P21可能参与乳腺癌的发生发展过程，检测P21在乳腺癌中的表达。有助于评估乳腺癌的预后，也为乳腺癌P2l的靶向治疗提供一定的参考价值。Objective To detect the expression of P21 in normal breast tissues, ductal carcinoma in situ （DCIS） and invasive breast cancer, respectively, and to explore its relationship with clinical characteristics, pathological markers, recurrence, metastasis, and prognosis of invasive breast cancer. Methods Immunohisto- chemical staining （SP） was used to detect the expression of P21 in 22 cases of normal breast tissues, 32 cases of DCIS, and 40 cases of invasive breast cancer. The correlation of their expressions with relevant clinical and pathological data was statistically analyzed. Results Expression of P21 in invasive breast cancer, DCIS, and normal breast tissues were gradually decreased. Expression of P21 in invasive breast cancer had statistical differ- ence with that in DCI$ and normal breast tissue. The difference had statistical significance （P〈0.05）. Expression of P21 was positively correlated with PR expression, and negatively correlated with tumor node metastasis （TNM） stage, histological grade and HER2 expression （P〈0.05）. Expression of I〉21 had no relation with vascular inva- sion, tumor size, age, menopausal status, ER or Ki-67 status. Conclusions Expression of P21 is related to the development, metastasis and prognosis of breast cancer. To detect the expression of P21 in breast cancer is help- ful in assessing the prognosis of breast cancer, and of value for targeted treatment on P21 of breast cancer.
