摘 要:目的探讨血管生成素样蛋白2(Angptl2)、转化生长因子β1(TGF-β1)与2型糖尿病(T2DM)大血管病变的相关性。方法于2015年1~12月,将84例T2DM患者分为单纯T2DM组(44例)和T2DM合并大血管病变组(40例),另将同期体检健康者45例纳入对照组。比较各研究组空腹血糖(FPG)、餐后2h血糖(2hFPG)、糖化血红蛋白(HbA1c)、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、空腹胰岛素(Fins)、胰岛抵抗指数(HOMA-IR)、收缩压(SBP)、舒张压(DBP)、C反应蛋白(CRP)、白细胞介素8(IL-8)、肿瘤坏死因子α(TNF-α)、Angptl2、TGF-β1水平。采用Logistic多因素回归分析与T2DM大血管病变相关的危险因素。结果T2DM合并大血管病变组患者年龄大于单纯T2DM组及对照组人群,病程较T2DM长,TC、LDL-C、HOMA-IR、SBP、CRP、IL-8、TNF-α、Angptl2、TGF-β1水平高于单纯T2DM组(P〈0.05)。Logistic多因素回归分析结果显示,年龄、糖尿病病程、TC、SBP、IL-8、TNF-α、Angptl2、TGF-β1与T2DM大血管病变相关(P〈0.05);平衡各项混杂因素后,Angptl2、TGF-β1仍与T2DM大血管病变相关(P〈0.05)。结论 Angptl2、TGF-β1是T2DM发病的独立危险因素之一,监测二者水平对预测T2DM大血管病变具有一定的意义。Objective To investigate the relationship between angiopoietin-like protein 2(Angptl2),transforming growth factor-β(TGF-β1)and type 2diabetes mellitus(T2DM)macrovascular diseases.Methods From Jan.to Dec.2015,84T2 DM patients were divided into simple T2 DM group(44cases)and T2 DM with macrovascular diseases group(40cases),and 45 healthy subjects were enrolled as control group.The levels of fasting blood glucose(FPG),2hpostprandial blood glucose(2hFPG),glycated hemoglobin(HbA1c),triglyceride(TG),total cholesterol(TC),low density lipoprotein(LDL-C),high density lipoprotein(HDL-C),fasting insulin(Fins),insulin resistance index(HOMA-IR),systolic blood pressure(SBP),diastolic blood pressure(DBP),C-reactive protein(CRP),interleukin-8(IL-8),tumor necrosis factor-α(TNF-α),Angptl2 and TGF-β1were compared between the three groups.The risk factors for T2 DM macrovascular diseases were analyzed by using Logistic multivariate analysis.Results Compared with simple T2 DM group and control group,the age of patients in T2 DM macrovascular diseases group were higher(P〈0.05).The course of diseases of T2 DM macrovascular diseases were longer than simple T2 DM group(P〈0.05).The levels of TC,LDL-C,HOMA-IR,SBP,CRP,IL-8,TNF-α,Angptl2,TGF-β1of T2 DM macrovascular diseases group were higher than simple T2 DM group(P〈0.05).Logistic regression analysis indicated that age,duration of diabetes,TC,SBP,IL-8,TNF-α,Angptl2 and TGF-β1were associated with T2 DM macrovascular diseases(P〈0.05).After balancing of confounding factors,Angptl2 and TGF-β1were still associated with T2 DM macrovascular diseases(P〈0.05).Conclusion Angptl2 and TGF-β1might be independent risk factors for the onset of T2 DM,which could be used for predicting T2 DM macrovascular diseases.
