摘 要:目的通过检测HBME-1、CK19和CD117在甲状腺乳头状癌、滤泡性腺瘤和桥本甲状腺炎组织学标本的表达,探讨3种抗体在甲状腺乳头状癌鉴别诊断中的临床价值。方法收集该科2013年1月至2015年12月85例甲状腺腺叶切除标本,进行HBME-1、CK19和CD117免疫组织化学染色,并对不同病变的染色结果进行分析。结果 HBME-1、CK19和CD117在甲状腺乳头状癌的阳性表达率分别为87.3%、98.2%和7.3%,三者阳性表达率在良性组与恶性组之间比较,差异有统计学意义(P<0.05),K值分别为0.582、0.551和0.874。结论甲状腺乳头状癌与良性病变的鉴别诊断,CD117检测优于HBME-1和CK19,实际工作中适于联合使用。Objective To explore the value of three antibodies in the differential diagnosis of papillary thyroid carcinoma,by detecting the expression of HBME-1,CK19 and CD117in papillary thyroid cacinoma,thyroid follicular adenoma and Hashimoto′s thyroiditis tissues.Methods Totally 85 cases were collected from January 2013 to December 2015,including papillary thyroid cacinoma,thyroid follicular adenoma and Hashimoto′s thyroiditis.They were immunohistochemical stained by HBME-1,CK19 and CD117.SPSS16.0software was used to analyze the relationship between the staining results with different pathological changes.Results The positive rates of HBME-1,CK19 and CD117were 87.3%,98.2%,and 7.3%,respectively.The positive expression of them in benign and malignant groups had significant difference(P〈0.05)and their consistency checking Kappa were 0.582,0.551,and 0.874,respectively.Conclusion In the differential diagnosis of papillary thyroid carcinoma and benign lesions,CD117 is better than HBME-1and CK19.It′s possible to use a combination of them in practice.
