子宫外子宫内膜间质肉瘤临床病理特点:11例分析

摘　　要：目的分析子宫外子宫内膜间质肉瘤的临床病理特点,及其与子宫内膜异位症的关系。方法回顾性分析北京协和医院2002年12月至2012年12月10年间收治的86例子宫内膜间质肉瘤(ESS)患者的临床病理资料,其中75例为子宫ESS,11例为子宫外ESS。结果与子宫ESS相比,子宫外ESS:(1)临床表现不同,最常见表现为盆腔包块(45.5%vs.10.7%,P<0.05);(2)术前血清CA125水平相当(中位值77.8U/L vs.24.9U/L,P>0.05);(3)合并子宫内膜异位症的可能性更大(36.4%vs.9.3%,P<0.05);(4)病理亚型及免疫组化特点无显著差异(P>0.05)。结论与子宫ESS相比,子宫外ESS有不同的临床特点,可能与子宫内膜异位症恶变相关。Objective： To analyze the clinical and pathological characteristics of extra-uterine endometrial stromal sarcoma（ESS）,and to explore its association with endometriosis. Methods： Eighty six patients received surgery at Peking Union Medical College Hospital from December 2002 to December 2012 because of endometrial stromal sarcoma. Among them. 75 patients suffered uterine ESS and 11 extra-uterine ESS. The medical records of those patients were collected and analyzed. Results. Compared with uterine endometrial stromal sarcoma,the patients with extra-uterine ESS had following features： （1）with different manifestations,and pelvic mass was common（45.5% vs. 10.70/oo ,P〈 0.05） ; （2）with similar presurgical CA125 level（median 77.8 U/L vs. 24.9 U/L,P〉0.05） ; （3）with a high rate of coexisting endometriosis （36.4 % vs. 9.3 %, P % 0.05 ） ; （4） with similar pathologic subtypes and immunohistochemical staining features（P〉0.05）. Conclusions： Extra-uterine endometrial stromal sarcoma has different clinical features from uterine endometrial stromal sarcoma,and it might be closely relevant to endometriosis.
