肾穿刺活检组织病理诊断中六胺银染色的改进

摘　　要：目的探讨肾穿刺活检组织病理诊断中六胺银染色染液配制法与染色方法改进的可行性。方法收集厦门大学附属第一医院病理科2014年1月~2015年12月确诊的60例肾穿刺活检病例的蜡块,每例蜡块均连续切片3片,随机分为3组。A组使用传统六胺银染液用传统染色法染色,B组使用改进六胺银染液用传统染色法染色,C组使用改进六胺银染液用改进染色法染色,以百分制评定180张六胺银染色片的得分,统计各组的得分,结果采用（x-±s）表示,采用独立样本t检验进行比较,分析各组的得分差异是否有统计学意义。结果 B组染色得分高于A组染色得分（82.3±10.4 vs.68.3±9.8,t=7.592,P〈0.001）;C组染色得分高于B组染色法得分（93.6±6.7 vs.82.3±10.4,t=7.076,P〈0.001）。结论改进六胺银染液配制方法及改进其染色方法,结果显示基底膜、网状纤维呈黑色,且界限清晰,免疫复合物呈红色,细胞核呈蓝色,背景粉红色,清晰无杂质,优于传统方法。Objective To explore the histopathological diagnosis of renal biopsy hexamine silver dyeing dye solution preparation method and the feasibility of improvement methods. Methods Collected in first affiliated hospital of xiamen university from January 2014 to December 2015,60 cases of renal biopsy diagnosis of wax,each wax from 60 cases of renal biopsy diagnosis all were cut into serial sections of three and randomly divided into three groups. group A was dyed by traditional hexamine silver dye solution under the traditional dyeing method,group B,which also under the traditional dyeing method,was dyed with improved hexamine silver dye solution. Under the improved dyeing method,group C was dyed with improved hexamine silver dye solution,scores of the groups（out of a possible 100）that were 180 pieces of hexamine silver staining sections,were presented in the form of（x-±s）. Compared with the independent sample t-test,to analyze the statistical significance of the differences in each group＇s scores. Results Group B had got significantly higher scores than group A（82.3±10.4 vs. 68.3±9.8,t=7.592,P 〈0.001）. While the scores of group C were much higher than group B（93.6±6.7 vs. 82.3±10.4,t=7.076,P 〈0.001）. Conclusion Under the improved hexamine silver dye solution preparation method and improved its dyeing method,the result shows that the basement membrane,reticular fiber in black,and clear boundaries,immune complex red,thenuclei are blue,pink background,clear and without impurities,in conclusion,the improved hexamine silver dye solution preparation method and improved dyeing method is significantly better than the traditional method.
