摘　　要：目的探讨缺血性脑卒中(CIS)合并糖尿病肾病患者预后的影响因素。方法对527例初发CIS合并糖尿病肾病患者进行研究,以CIS复发或因病死亡为研究终点,分为预后不良组(达到终点事件者)58例和预后良好组(未达到终点事件者)469例,比较两组患者临床特征的差异,采用logistic回归分析法探讨预后的影响因素。结果两组患者年龄、入院改良Rankin评分(m RS)、美国国立卫生研究院卒中评分(NIHSS)、日常生活能力量表(ADL)、糖化血红蛋白、血肌酐、尿蛋白/肌酐比比较,差异均有统计学意义(均P<0.05)。logistic回归分析显示,入院m RS、NIHSS、ADL、尿蛋白/肌酐比、血肌酐和糖化血红蛋白为CIS合并糖尿病肾病患者预后的影响因素(均P<0.05)。结论入院mRS、NIHSS、ADL、尿蛋白/肌酐比、血肌酐和糖化血红蛋白为CIS合并糖尿病肾病患者预后的影响因素。】Objective To investigate the influencing factors of prognosis in diabetic nephropathy patients with cerebralischemic stroke （CIS）. Methods A total of 527 patients with CIS and diabetic nephropathy from 4 hospitals in Zhejiang provincewere included in the study. Endpoint of the study was defined as CIS recurrence or death due to illness. There were 58 patientswith poor prognosis （reach the end point） and 469 with good prognosis （not reach the end point）. The differences of clinicalfeatures were compared between the two groups, and the influencing factors of prognosis were analyzed by logistic regressionanalysis. Results There were significant differences in the age, Modified Rankin Scale （mRS）, National Institute of Health StrokeScale （NIHSS） and Activity of Daily Living Scale （ADL） scores, glycosylated hemoglobin, serum creatinine, urinaryprotein/creatinine levels between two groups （P〈0.05）. Logistic regression analysis showed that mRS, NIHSS and ADL scores,urinary protein/creatinine, serum creatinine and glycosylated hemoglobin levels were the influencing factors of prognosis indiabetic nephropathy patients with CIS （P〈0.05）. Conclusion mRS, NIHSS and ADL scores, urinary protein/creatinine, serumcreatinine and glycosylated hemoglobin levels would influence the prognosis in diabetic nephropathy patients with cerebral ischemicstroke.【
