单、双相抑郁障碍患者GRmRNA的表达水平及ACTH和血清皮质醇的浓度关系分析

摘　　要：目的研究单、双相抑郁障碍患者GRmRNA的表达水平及促肾上腺皮质激素释放激素（ACTH）和血清皮质醇的浓度的关系。方法选取2014年10月～2015年10月来我院医治的单相、双相抑郁障碍患者共84例为研究对象。根据抑郁障碍诊断分为单相抑郁障碍组（UDD组）及双相抑郁障碍组（BDD组）。另选同时期于我院进行体检的正常人作40例为对照组，研究各组GRmRNA、ACTH及皮质醇水平差异。结果患者组及对照组外周血单个核细胞中GRα mRNA水平具有显著性差异（P〈0．05）。UDD组及BDD组中轻中度组、重度组和对照组的GR仅mRNA水平具有显著性差异（P〈0．05），但重度组GRα mRNA水平显著弱于轻中度组（P〈0．05）。患者组及对照组ACTH浓度未见统计学差异（P〉0．05），皮质醇浓度具有统计学差异（P〈0．05），且随着病情的加重，血清皮质醇浓度呈上升的趋势（P〈0．05）。患者组及对照组GRα的亚细胞分布及核内表达水平差异有统计学意义（P〈0．05），且重度组核内分布显著弱于轻中度组。患者组HAMD总分均与GRα mRNA的表达呈负相关（r=-0．746，P〈0．05；r=-0．831，P〈0．05）；患者组中均未出现HAMD与ACTH、皮质醇浓度的相关性以及GRα mRNA与ACTH及皮质醇浓度的相关性（P〉0．05）。结论单、双相抑郁患者外周血GRmRNA的表达水平、皮质醇浓度与病情的严重程度密切相关，但单、双相抑郁患者组间未见统计学差异。糖皮质激素受体可能仅为抑郁障碍的的指标之一，在单、双相抑郁的发病过程起到十分重要的作用。Objective To investigate the relationship relationship among GRmRNA and serum level of ACTH and serum level of cortisol in patients with single and double phase depression. Methods 84 cases of unipolar and bipolar depression patients were selected as study subjects. GRmRNA, ACTH and cortisol levels in serum were examinated. Results Patients group and control group in peripheral blood mononuclear cells GR mRNA level has significant difference （P〈 0.05）. UDD group and BDD group in light moderate group had significant difference （P〈 0.05）, and the severe group of GR mRNA level was weaker in light than the moderate group, cortisol concentration has significant difference （P〈 0.05）.HAMD scores in patients group and GR mRNA expression were negatively correlated （ r=-0.746, P〈 0.05; r=-0.831, P〈 0.05）; there wash＇ t significant relationship between the HAMD and serum ACTH level in patients group, also cortisol concentration correlation and GR mRNA and adrenocorticotropic hormone （ACTH） and cortisol concentration correlation （P 〉 0.05）. Conclusion The expression level of GRmRNA in peripheral blood of patients with depression and bipolar depression was closely related to the severity of the disease, but there was no statistical difference between the two groups.
