外用重组人粒细胞巨噬细胞刺激因子凝胶对肛瘘术后创面愈合的作用

摘　　要：为探讨外用重组人粒细胞巨噬细胞刺激因子（rhGM—CSF）凝胶对肛瘘术后创面愈合的作用，将低位单纯性肛瘘术后60例患者随机分为治疗组和对照组，每组各30例。治疗组术后创面采用外用重组人粒细胞巨噬细胞刺激因子凝胶换药，对照组采用甲硝唑引流条换药。术后1、7、14、21d计算创面愈合率，测定创面组织羟脯氨酸含量、CD31表达情况和微血管密度。结果显示，术后14d和21d，治疗组创面愈合率分别为（66．98±7．83）％和（92．61±4．83）％，较对照组同期创面愈合率的（48．29±6．64）％和（78．31±7．34）％均有明显提高（均P〈0．05）。术后14d创面中羟脯氨酸含量治疗组[（5．05±1．00）mg／g]较对照组[（3．13±0．54）mg／g]明显升高（P〈0．05）。术后14d创面中CD31表达量和创面微血管密度，治疗组（120．06±4．37）显著高于对照组的（79．33±4．22），P〈0．05。结果表明，重组人粒细胞巨噬细胞刺激因子凝胶用于肛瘘术后换药，可以加速创面愈合。This study was to explore the effect of external using recombinant human granulocyte-macrophage colony stimulating factor（rhGM-CSF） gel on wound-surface healing after anal fistula surgery,divided 60 cases after the surgery of low simple anal fistula into treatment group and control one,30 cases for each. The former received external using rhGM-CSF gel for dressing exchange on wound after surgery; the latter was given metronidazole drainage strip for dressing exchange; then calculated the wound-surface healing rate and measured the hydroxyproline content of wound tissue, the expression condition of CD31 and the density of microvessel on the 1st, 7th, 14th, 21st d after surgery.As results, on the 14th and 21st after surgery,wound-surface healing rate of the former （66.98±7.83,92.61±4.83） % was obviously higher than that of the latter （48.29±6.64,78.31±7.34）% at the same time （all P〈0.05）.On the 14th day after surgery,in hydroxyproline content,the former [（5.05±1.00） mg/g] was significantly higher than the latter [（3.13±0.54） mg/g, P %0.05]; On the 14th day after surgery,in the expression quantity of wound CD31 and the density of wound microvessels,the former （120.06±4.37） was significantly higher than the latter （79.33±4.22） （ P %0.05）. In conclusion,rhGM-CSF gel for dressing exchange after anal fistula surgery can accelerate the wound-surface healing.
