摘　　要：目的探讨数字化三维立体定位活检在触诊阴性乳腺病变中的诊断价值。方法分析2014年10月-2015年10月在江苏省中医院就诊的40例触诊阴性乳腺病变患者的临床资料及影像学表现。所有病灶的均按照BI-RADS评分体系进行分类界定,再对病灶进行立体定位穿刺,并与手术活检病理对照。结果共有40例患者,48个病灶进行三维立体定位活检,定位时发生迷走神经反应的1例,所有病灶均一次定位、切除成功,无术后并发症。病理证实恶性病变10个(20.83%),癌前病变7个(14.58%),其他良性病变31个(64.58%)。结论三维立体定位活检对触诊阴性乳腺病变定位准确,操作安全简便,为临床微创手术提供准确的指导,是诊断触诊阴性乳腺病变的有效方法。Objective To explore the value of digital mammary gland three-dimensional wire-localization in diagnosis of non-palpable breast lesions. Methods The clinical data and imaging findings of 40 patients with non-palpable breast lesions from the hospital from Octo- ber 2014 to October 2015 were analyzed retrospectively. All the breast lesions were categorized according to BI-RADS rating system, then three-dimensional wire- localization and puncture were performed, the results were compared with pathological results after biopsy. Results A total of 48 breast lesions underwent digital mammary gland three-dimensional wire-localization and biopsy, Vagal response oc- curred in one patient, all the breast lesions were located and reseeted successfully for one time, no postoperative complications occurred. Ten breast malignant lesions （20. 83% ）, 7 breast precancerous lesions （ 14. 58% ）, and 31 breast other benign lesions （64. 58% ） were con- firmed by pathological examination. Conclusion Digital mammary gland three-dimensional wire-localization and biopsy is accurate for pa- tients with non-palpable breast lesions, the operation is safe and simple, which provides accurate guidance for clinical minimally invasive surgery, so it is an effective method in diagnosis of non-palpable breast lesions.
