术前Onyx胶超选择性栓塞供血丰富脑膜瘤的作用

摘　　要：目的探讨应用Onyx胶术前超选择性栓塞供血丰富脑膜瘤的手术效果。方法回顾性分析苏州大学附属第三医院（江苏省常州市第一人民医院）神经外科2013年5月至2016年4月收治的行术前肿瘤栓塞的供血丰富脑膜瘤患者16例,对肿瘤的栓塞程度、手术时间、出血量以及是否有并发症等方面进行分析结果 16例患者中,完全栓塞1例、大部分栓塞6例、部分栓塞6例、少量栓塞3例。术中出血量（250±65）ml,手术时间中位数为2.6（2.2-3.0）h。肿瘤切除程度：SimpsonⅠ级12例,Ⅱ级4例。无颅内出血、脑梗死及血管夹层并发症。结论使用Onyx胶超选择性栓塞供血丰富脑膜瘤有利肿瘤切除,减少并发症。Objective To investigate the operation effect of preoperative superselective embolization of hypervascular meningioma with Onyx. Methods Sixteen patients with hypervascular meningioma admitted to the Department of Neurosurgery,the Third Affiliated Hospital（ Changzhou First People＇ s Hospital of Jiangsu） and treated with preoperative tumor embolization from May 2013 to April 2016 were analyzed retrospectively. The degree of tumor embolization,operation time,bleeding volume,and whether having complications were analyzed. Results In 16 patients,the complete embolization was in 1 case,subtotal embolization was in 6 cases,partial embolization was in 6 cases,and small amount of embolization was in 3 cases. The intraoperative blood loss was 250 ± 65 ml. The median operative time was 2. 6（ 2. 2-3. 0） h.The degree of tumor resection： the Simpson gradeⅠin 12 cases andⅡin 4 cases. No intracranial hemorrhage,cerebral infarction,and vascular dissection complications were observed. Conclusion Superselective embolization of hypervascular meningioma with Onyx is conducive to tumor resection and can reduce the complications.
