超声造影联合荧光血管造影在脉络膜肿瘤诊断中的价值

摘　　要：目的探讨超声造影及荧光血管造影在脉络膜肿瘤诊断中的价值。方法脉络膜血管瘤36例,脉络膜黑色素瘤22例,脉络膜转移癌17例,经手术病理或临床综合诊断及随访证实,对其行超声造影及荧光血管造影检查。结果脉络膜血管瘤、脉络膜黑色素瘤与脉络膜转移癌超声造影在峰值强度（IMAX）、10%~90%峰值强度的上升时间（RT）、达峰时间（TTP）及平均渡越时间（m TT）的差异均有统计学意义（P〈0.05）。脉络膜血管瘤荧光造影表现为早期显影,瘤体呈不规则丛状或斑状的强荧光,中期荧光逐渐增强,晚期有不同程度荧光渗漏及瘤体染色。脉络膜黑色素瘤荧光造影表现为早期呈弱荧光或低荧光,中期荧光逐渐增强,晚期瘤体多呈斑状高荧光伴不同程度荧光渗漏。脉络膜转移癌荧光造影表现为造影早期瘤体呈斑驳样弱荧光,中期逐渐转变为强荧光,晚期增强呈斑驳样荧光。结论超声造影及荧光血管造影检查在脉络膜肿瘤的诊断和鉴别诊断中具有重要价值。Objective To investigate the diagnostic value of using contrast-enhanced ultrasound（CEUS）and fluorescein angiography in choroidal tumors. Methods Conventional CEUS and fluorescein angiography were performed in 36 cases of choroidal hemangioma,22 cases of choroidal melanoma,and 17 cases of choroidal metastasis,which were confirmed by surgical pathology or clinical diagnosis and follow-up. Results There was difference in CEUS among of choroidal hemangioma,choroidal melanoma and choroidal metastasis in IMAX,RT, TTP and m TT（P 0. 05）. The fluorescein angiography imaging of choroidal hemangioma showed irregular plexiform or patchy strong fluorescence in tumor in early stage, the fluorescence increased gradually in medium stage, at the last there were degrees of fluorescence leakage and tumor staining. The fluorescein angiography imaging of choroidal melanoma showed weak fluorescence or low intensity fluorescence in early stage, the fluorescence increased gradually in medium stage, at the last the tumor showed patchy strong fluorescence with different degrees of fluorescence leakage. The fluorescein angiography imaging of choroidal metastasis showed patchy weak fluorescence in early stage, the fluorescence gradually transformed into strong fluorescence in medium stage, at the last the fluorescence increased to patchy form. Conclusion The CEUS and fluorescein angiography are valuable in the diagnosis and differential diagnosis of choroidal tumors.
