肝巨大血管瘤射频消融术后疗效观察

摘　　要：目的：观察B超引导下经皮射频消融（RFA）治疗肝血管瘤的安全性和近期疗效。方法：收集笔者所在单位2008年7月-2011年3月在B超引导下经皮穿刺集束射频治疗肝血管瘤共52例,64个病灶,根据血管瘤直径大小,将其分为5～10 cm组及〉10 cm组,术后通过超声造影及CT增强扫描评价RFA疗效。结果：射频消融治疗的64个病灶,平均直径（7.9±2.3）cm;肿瘤直径5～10 cm组59个病灶,平均直径（7.1±1.6）cm;〉10 cm组5个病灶,平均直径（16.9±1.2）cm;均顺利完成RFA治疗。术后随访3个月,肿瘤直径平均缩小73.1%（57.8%～87.2%）,直径5～10 cm组,完全缓解49个病灶,部分缓解10个病灶;直径〉10 cm组,完全缓解1个病灶,部分缓解4个。总缓解率100%。所有病例无大出血、膈肌损伤、气胸、腹膜炎、肝脓肿、胆漏等严重并发症发生。术后随访4年以上,有4例瘤体有再生长,复发率为7.6%。结论：在掌握好技术规范的前提下,RFA治疗肝血管瘤安全、可靠、复发率低。可作为肝血管瘤治疗的主要方法。Purpose： To evaluate the efficacy and safety of radiofrequency ablation （RFA） under ultrasound guidance for the treatment of hepatic hemangiomas.Methods：Fifty-two patients with hepatic hemangiomas （64 lesions） received RFA treatment under ultrasound guidance from Jul. 2008 to Mar. 2011 in our hospital. According to the diameter of the hemangiomas, 64 lesions were divided into 5-10cm group and 〉10cm group. After the operation, RFA efficacy was evaluated with ultrasound and enhanced CT scanning. Results：The mean diameter of the 64 lesions was （7.9±2.3）cm; 59 lesions were in the 5-10cm diameter group, the mean diameter of them was （7. 1±1.6）cm; 5 lesions were in the 〉 10 cm group, the mean diameter of them was （16.9±1.2）cm; RFA was successfully completed. Patients were followed up for 3 months, the average tumor diameter was reduced 73.1% （57.8%-87.2%）; 49 lesions were complete remission and 10 lesions were partial remission in 5-10 cm diameter tumor group; 1 lesions were complete remission and 4 lesions were partial remission in 〉10cm diameter tumor group. The overall response rate was 100%. No serious complication occurred. The recurrent rate was only 7.6% after four years. Conclusions：RFA is an effective and safe method for the treatment of hepatic hemangiomas.
