摘 要:目的:评估与比较MRI引导和超声引导高强度聚焦超声(HIFU)完全消融子宫肌瘤的治疗能量效率、时间效率和临床安全性。方法:回顾性分析MRI引导高强度聚焦超声(MRg HIFU)治疗10例13个完全消融子宫肌瘤和超声引导高强度聚焦超声(USg HIFU)治疗22例28个完全消融子宫肌瘤的临床资料和治疗数据,评估和比较这两种影像方式引导HIFU治疗子宫肌瘤的治疗时间和速度、超声辐照时间、超声释放功率和能量、能效因子和治疗后不良事件及并发症的发生。结果:MRg HIFU和USg HIFU完全消融子宫肌瘤的治疗时间、超声辐照时间、超声治疗功率、超声治疗剂量、能效因子(EEF)及治疗速度分别为174.5±42.2min和114.4±39.2min,24.7±9.0min和19.4±6.8min,310.2±62.5W和391.6±16.6W,483.0±248.2 k J和463.2±156.4k J,5.1±3.0k J/cm3和6.8±5.2 k J/cm3及42.2±25.6 cm3/h和70.9±41.9cm3/h。两组治疗后均未发生严重不良事件和并发症。结论:超声引导和磁共振引导HIFU完全消融子宫肌瘤均是安全的,且超声治疗能量效率两者差异无显著性,但USg HIFU在治疗时间效率上优于MRg HIFU。Purpose: To compare the treatment energy and time efficiency, safety between MR-guided and ultrasound-guided high intensity focused ultrasound (MRgHIFU, USgHIFU) for complete ablation of fibroids. Methods: The sonication parameters and treatment data from 13 uterine fibroids in 10 women treated with MRgHIFU and 28 uterine fibroids in 22 women treated with USgHIFU were analyzed retrospectively. The treatment and sonication time, sonication power and released energy, the energy efficiency factor (EEF) and treatment speed were compared between MRgHIFU and USgHIFU, while the adverse events and complications were also assessed. Results: The treatment time was 174.5±42.2minutes and 114.4±39.2 minutes, the sonication time was 24.7±9.0 minutes and 19.4±6.8 minutes, the sonication power was 310.2±62.5W and 391.6±16.6W, the sonication energy was 483.0±248.2 KJ and 463.2±156.4KJ, EEF was 5.1±3.0 KJ/cm^3 and 6.8±5.2 KJ/cm^3 and the mean treatment speed was 42.2±25.6 cm^3/h and 70.9±41.9 cm^3/h in the MRgHIFU and USgHIFU treatment for complete ablation of fibroids, respectively. There was no severe adverse event and major complication in the both groups after treatment. Conclusions: MRgHIFU and USgHIFU were both safe for complete ablation of fibroid without major complication. The USgHIFU was superior to MRgHIFU in the time efficiency of treatment.
