摘 要:目的:观察肠炎康对溃疡性结肠炎(UC)的临床疗效及通过其对肠黏膜组织中白细胞介素-2(IL2)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)表达的影响,探讨其治疗UC的细胞因子作用机制。方法:通过症状、肠镜检查并结合活检结肠黏膜病理检查结果,明确诊断为UC的60例处于活动期的患者,采用随机、对照的研究方法,随机分组,每组30例,其中治疗组应用肠炎康,对照组应用美沙拉嗪缓释颗粒(MS),疗程为8周,对两组疗效进行评价,并用随机数字表法从治疗组中随机抽取10例患者,应用免疫组化法检测其病变肠道黏膜中IL2、IL-6、TNF-α表达的变化。对以上观察及检查、检验结果进行详细记录,并均采用积分综合计量,应用自身前后对照及组间对照的方法,进行统计分析。结果:肠炎康治疗组与美沙拉嗪(MS)对照组,在总有效率上比较,治疗组的总有效率高于对照组,但肠炎康组与MS组疗效之间差异无明显统计学意义(P>0.05);通过Wilcoxon符号秩和检验的结果提示:按照治疗后-治疗前的负秩输出,P均<0.05,说明IL-2、IL-6及TNF-α表达水平在应用肠炎康治疗前、后差异具有统计学意义。结论:肠炎康治疗有效,与MS疗效相似,且从细胞因子机制角度得出,肠炎康可较好的抑制肠黏膜促炎细胞因子表达,而发挥保护肠黏膜的作用。Objective: To observe the clinical efficacy of Chang Yan Kang Decoction (Chang Yan Kang, CYK) in treating of ulcerative colitis (UC) and the effect of interleukin -2, interleukin -6 and tumor necrosis factor alpha (TNF -α) expression in the intestinal mucosa, and to investigate the mechanism of the therapeutic effect on the treatment of UC. Methods : By symptoms, endoscopy, biopsy and pathological examination results of colonic mucosa, confirm the diagnosis of UC 60 patients in active stage were treated by randomized, controlled study method, were randomly divided into two groups, 30 cases in each group, the treatment group was treated with Chang Yan Kang, the control group was treated with Mesalazine SR Granules (MS)treatment, treatment for 8 weeks. The efficacy of the two groups was evaluated, and using a random number table method to randomly selected 10 patients from treatment group. Immunohistochemical method to detect changes to the expression of IL - 2, IL - 6 and TNF - α in the lesions of intestinal mucosa. The results of the a- bove observation and inspection, inspection, and the use of integrated measurement, the application of their own before and after the control and group control method, statistical analysis. Results :The total effective rate of the treatment group was higher than that of the control group, but there was no significant difference between treatment group and control group ( P 〉 0.05 ). The results of the Wilcoxon signed rank sum test are suggested : according to the negative rank of the difference of treatment before and after treatment, P 〈 0.05, the expression levels of IL - 2, IL - 6 and TNF -α werestatistically significant in the treatment before and after the application of the treatment of enteritis.
