青岛市老年人群肝功能异常与代谢综合征关系的研究

摘　　要：目的研究老年人肝功能异常与代谢综合征的关系。方法对2013年参加健康体检的21 928例65岁以上老年人的体检资料进行分析。分别测量身高、体重、血压,检测血脂、血糖等生化指标。血清丙氨酸转氨酶〉50 U/L视为肝功能异常。结果肝功能异常检出率为2.77%;高血压、高血糖、高甘油三酯、超重或肥胖和代谢综合征的患病率分别59.57%、23.70%、30.77%、39.57%和20.20%,除高血压在肝功能正常组与异常组差别无统计学意义外,其他肝功能异常组各指标患病率均高于肝功能正常组（P≤0.01）;调整年龄和性别变量后的多因素回归分析提示,肝功能异常组患代谢综合征的危险性是肝功能正常组的1.948倍,95%CI为1.633～2.324。结论高血糖、高甘油三酯、超重或肥胖等代谢综合征的主要构成组分与肝功能异常相关。Objective To study the relationship between abnormal liver function syndrome and metabolic syndrome among the elderly. Methods The health examination results in 2013 were analyzed, of which 21928 elderly residents aged over 65 years old were involved. Body height, body weight, blood pressure, biochemistry items including alanine aminotransferase,fasting blood glucose, high-density lipoprotein cholesterol, triglycerides were detected. Subjects with elavated serum alanine aminotransferase（50U/L） were considered to have abnormal liver function. Results The prevalence of abnormal liver function was 2.77%. The prevalence of hypertension, high serum glucose, high triglycerides, overweight and metabolic syndrome were 59.57%, 23.70%, 30.77%, 39.57% and 20.20%, respectively, in addition to hypertension in liver function normal group and abnormal group, the difference was not statistically significant, other indexes prevalence rate of liver function abnormal group were higher than those in the normal group（P ≤0.01）. The results of multivariate logistic regression analysis after adjusted by age and gender showed that the OR value for metabolic syndrome in subjects with abnormal liver function was as 1.948 times as in normal subjects.（95%CI： 1.633～2.324）. Conclusion High serum glucose,high triglycerides and overweight are the main risk factors for metabolic syndrome associated with abnormal liver function.
