摘　　要：目的:探究纤维桩与铸造金属桩核对较大面积缺损患者的患牙修复效果及分析预后的影响。方法:回顾性分析2012年1月至2015年6月解放军总医院就仍的240例大面积牙体组织缺损患者(患牙292颗)临床资料,按照治疗时不同修复方法分为两组,研究组采用纤维桩修复患者132例(患牙156颗),对照组采用铸造金属桩核修复患者108例(患牙136颗),对比两组患牙修复效果和预后情况。结果:研究组患牙修复后的牙龈着色、冠核松脱、牙跟折裂等失败率显著低于对照组,(P<0.01);同时研究组预后效果优于对照组,即牙周炎、牙龈炎发生率均未超过5.00%,低于对照组(P<0.01);研究组预后生活质量评分均优于对照组,比较差异显著(P<0.05)。结论:纤维桩对较大面积缺损患者的患牙修复效果及预后均显著优于铸造金属桩核修复治疗,具有临床实际应用价值。Objective： To compare the effect of fiber post and cast post core on restorations of large tooth defects. Methods：292 teeth with large tooth defects from of 240 patients were chosen, and divided into two groups according to treatments with fiber post or cast post. All of them were treated in our hospital from January 2012 to June 2015. The 156 teeth from132 patients were treated with fiber post as study group, and the 136 teeth from 108 patients was treated with cast metal post core as control group. The failure（including discoloration of marginal gingiva, loosening of crown or core, root fracture etc）and were evaluated. Results： The study group shows significant lower failure rate than the control group（P 0.01）. The dental plaque index, bleeding index and loosening index score were decreased in both groups. Incidences of periodontitis and gingivitis in the study group show not more than 5.00%, and were significantly lower than those in the control group（P〈0.05）. Conclusion： Compared with cast metal post core, fiber post performs better on restoration of large tooth defects.
