康复医学发展现状调查与对策研究——以宁夏银川市为例

摘　　要：目的了解宁夏回族自治区银川市当前康复医疗机构、康复人才及康复需求现状。方法 2015年6-9月调查银川市45所综合性医院,采用自行设计的＂康复机构调查表＂,调查内容包括医院的康复医学科设置、康复床位分布、康复门诊日流量等项目。采用自行设计的＂康复医技人员调查表＂,调查内容包括康复医技人员的职称构成、康复医技人员的学历构成等项目。对兴庆区、金凤区、西夏区、永宁县、贺兰县5个地区的居民进行社区康复需求调查,采用分层整群随机抽样的方法抽取社区居民1 260名,均自愿接受调查。采用自行设计的＂社区居民康复需求表＂,调查内容包括对康复服务的需求、康复需求的项目等。结果 （1）全市45所各级各类医院中设置康复医学科的有11所（24.4%）,一、二级医院康复学科设置、康复病床数、康复门诊日流量显著低于三级医院,差异均有统计学意义（P〈0.01）。（2）全市45所各级各类医院中,共有康复医技人员110名,一、二级医院康复医技人员高、中、低级职称及研究生、本科学历人数均低于三级医院,而一、二级医院康复医技人员本科以下学历人数显著高于三级医院,差异均有统计学意义（P〈0.05或0.01）。（3）纳入社区居民1 260名,获得有效问卷1 204份,合格率为95.6%;5个地区居民中慢性常见病康复服务的需求率为90.0%（1 084/1 204）。结论目前银川市各级各类医院对康复科的投入不足,康复医学在服务能力及人才培养等方面严重不足,且存在地区分布和发展不均的状况,与地区康复需求高严重不符。Objective To investigate the status quo of the current rehabilitation medical institutions,rehabilitation talents and demands in Yinchuan City of the Ningxia Hui Autonomous Region. Methods Forty-five comprehensive hospitals of Yinchuan City were investigated by adopting the self-designed Rehabilitation Institutions Questionnaire from June to September2015,the investigation contents included the items such as the setting of rehabilitation medicine department,rehabilitation beds distribution and daily flow of outpatients. The self designed the Rehabilitation Medical and Technical Staff Questionnaire was adopted,the investigation contents included the items such as the professional tittles composition and education composition of rehabilitation medical and technical staffs. The community rehabilitation demands were investigated among the residents in 5 dis-tricts of Xingqing District,Jinfeng District,Xixia District,Yongning County and Helan County. A total of 1 260 residents were extracted by adopting the stratified cluster random sampling method and voluntarily received the investigation. The self-designed Community Rehabilitation Demands Table was adopted and the investigation content included the need for rehabilitation service and rehabilitation demand,etc. Results（1）Among 45 different levels and different types of hospitals in the whole city,11 hospitals（24.4%） set the rehabilitation medicine department,the setting of rehabilitation department,rehabilitation beds number,daily flow of outpatients in the primary,second-class hospitals were significantly lower than those in the tertiary hospitals,the differences were statistically significant（P〈0.01）.（2）Among 45 different levels and different types of hospitals in the whole city,there were 110 rehabilitation medical and technical staffs,the number of high,middle and low professional titles,postgraduates and undergraduates in the primary,second-class hospitals was lower than that in the tertiary hospitals,but the number the rehabilitation medical
