摘  要:目的探讨帕金森病(PD)伴发轻度认知功能障碍(MCI)患者血浆磷脂(AP)含量变化及意义。方法入选PD患者70例,同时入选年龄、性别相匹配的40例健康人作为对照组,记录PD患者的性别、年龄、病程、受教育年限、Hoehn&Yahr(H-Y)分级,采用蒙特利尔认知评估量表(Mo CA)对所有研究对象认知功能进行评估。采用层析技术结合改良的无机磷定量方法检测血浆AP含量,并进行比较分析。结果 PD组血浆AP水平明显高于对照组[(8.17±1.89)U/L比(4.64+1.74)U/L,P<0.01],早期PD组与中晚期PD组血浆AP水平比较差异无统计学意义(P>0.05),合并MCI的PD组血浆AP水平高于无MCI的PD组[(9.37±2.83)U/L比(7.12±2.76)U/L,P<0.05];PD患者Mo CA评分与受教育年限(B=0.431,P<0.01)呈正相关、与血浆AP水平(B=-0.617,P<0.01)、病程(B=-0.413,P<0.05)和H-Y分级(B=-0.332,P<0.05)呈负相关。结论 PD患者存在细胞膜损伤,血浆AP升高与PD患者的MCI具有相关性,其有望成为PD合并MCI患者潜在的早期血浆生物标记物。Objective To study the change of plasma level of acid phospholipid( AP) in patients with Parkinson's disease( PD),and to explore its association with cognition function. Methods Seventy patients with PD,and 40 ageand gender-matched healthy subjects were enrolled. The information including gender,age,illness duration,years of education and Hoehn Yahr( H-Y) stage were recorded.The cognitive function of all the patients with PD and the healthy controls were measured by using Montreal Cognitive Assessment( Mo CA) scale. The plasma level of AP was detected by measuring its inorganic phosphorus after separation by chromatograph. Results Plasma level of AP in PD group was significantly higher than that in control group( 8. 17±1. 89 U / L vs. 4. 64±1. 74 U / L,P0. 01). Plasma level of AP showed no significant differences between early PD group and middle-late PD group( P0. 05). Plasma level of AP in PD group with mild cognitive impairment( MCI) was obviously higher than that in PD group without MCI( 9. 37 ± 2. 83 U / L vs.7. 12±2. 76 U / L,P0. 05). In PD patients,Mo CA score was positively correlated with years of education( B = 0. 431,P0. 01),and was negatively correlated with plasma level of AP( B =-0. 617,P0. 01),and illness duration( B =-0. 413,P0. 05),and H-Y stage( B =-0. 332,P〈0. 05). Conclusions The cell membrane suffered from serious damage in patient with Parkinson's disease. Increased plasma levels of AP may be associated with MCI in PD patients.Plasma levels of AP may be a potential early predictive index in PD patients with cognitive impairment.
