胸部放疗在广泛期小细胞肺癌治疗中的作用

摘　　要：目的:探讨胸部放疗在广泛期小细胞肺癌治疗中的作用。方法:广泛期小细胞肺癌患者112例。其中化疗+放疗组69例,单纯化疗组43例。胸部放疗剂量:<50Gy 23例,≥50Gy 46例。比较化疗+放疗组和单纯化疗组的治疗效果。结果:全组近期疗效:CR 14.3%(16/112),PR 59.8%(67/112)。生存率:1年49%,2年19%,中位生存期1 0.3个月。化疗+放疗组6 9例,疗效评价:CR 1 9.8%,PR 6 9%,1年生存率52.9%,2年生存率18.8%,中位生存期11个月。单纯化疗43例,疗效评价:CR 6.8%,PR 47%,1年生存率28.9%,2年生存率7.8%,中位生存期6个月。结论:胸部放疗在广泛期小细胞肺癌治疗中可以较好的缓解症状和体征、改善生存质量和延长生存时间。Objective：To evaluate the role of thoracic radiotherapy in the treatment for patients with extensive stages of small cell lung cancer. Methods：All 112 patients with ED - SCLC were treated,69 patients were chemotherapy＋ radiotherapy,43 patients were chemotherapy alone. 23 patients of the total dose was less than 50Gy,the total dose of 46 cases was more than 50Gy thoracic radiotherapy. Compare the effect in chemotherapy ＋ radiotherapy with chemotherapy alone. Results：The complete response and partial response of overall group were 14. 3% and 59. 8% . The 1 -and 2 - year survival rates were 49% and 19% ,the median survival was 10. 3 months. Chemotherapy ＋ radiotherapy group was 69 patients,CR and PR was 19 . 8 % and 69 % . The 1 - and 2 - year survival rates were 52. 9% and 18. 8% ,the median survival was 11 months. Chemotherapy group was 43 patients,CR and PR was 6. 8% and 47% . The 1 - and 2 - year survival rates were 28. 9% and 7. 8% ,the median survival was 6 months. Conclusion：It is effective with radiotherapy to palliative symptoms and prolong survival in patients with extensive stage small cell lung cancer.
