高危型HPV检测在新疆宫颈癌筛查中的应用价值

摘　　要：目的评价高危型HPV基因型检测（Cobas HPV）在新疆宫颈癌筛查中的应用价值。方法对2014年09月~2015年8月在新疆自治区人民医院住院及门诊就诊行HC2、TCT检测、并有病理诊断结果的564位女性的宫颈细胞标本进行高危型HPV基因型检测。结果 HC2阳性率为83.86%,Cobas HPV阳性率为64.19%,TCT阳性率为61.52%;HC2诊断的灵敏度为94.4%（118/125）,特异度为19.1%（84/439）;Cobas HPV诊断的灵敏度为88.0%（110/125）,特异度为42.6%（187/439）,TCT诊断的灵敏度为54.4%（68/125）,特异度为36.4%（160/439）。不同液基细胞学分级HR-HPV感染率不同,差异具有统计学意义（P〈0.05）,Cobas HPV与HC2总符合率为78.50%（Kappa=0.469）。结论 Cobas HPV在诊断CIN2＋病变时优于HC2、TCT检测,Cobas HPV检测对ASCUS诊断分流优于HC2,Cobas HPV检测在新疆宫颈癌筛查中具有较高的临床应用价值。Objective The main purpose of the passage is to evaluate the application value of high-risk HPV genotype test during the cervical cancer screening. Methods High-risk genotype tests were carried out for cervical cell samples in 564 female patients who were performed the cervical cancer screening as well as HC2 and TCT test with pathological results in Xinjiang Uygur Autonomous Region between September 2014 and August 2015. Results The positive rate of HC2 was 83.86%,which of COBAS HPV was 64.19% and TCT was 61.52%. Regarding the detection of CIN2＋ the sensitivity and specificity were 94.4%（118/125）and 19.1%（84/439）for HC2,and88.0%（110/125）and 42.60%（187/439）for Cobas HPV, and 54.4%（68/125）and 36.4%（160/439）for TCT.HR-HPV infection rate is varying in different TCT classification.The difference has statistical significance（P〈0.05）. The crude agreement between Cobas HPV and HC2 is 78.50%（Kappa=0.469）. Conclusion In the diagnosis of CIN2＋, high-risk HPV genotypes is better than HC2, TCT. High-risk HPV genotype test which has higher clinical value in the screening of cervical cancer in the Xinjiang region is superior to HC2 and TCT.
