超声引导下粗针穿刺组织病理活检检测乳腺癌患者人表皮生长因子受体-2表达的价值

摘　　要：目的探讨超声引导下粗针活检组织病理检查诊断乳腺癌的价值及乳腺癌组织中人表皮生长因子受体-2（human epidemic growth factor receptor-2，HER-2）基因表达情况。方法36例乳腺癌患者均行超声引导下乳腺肿块穿刺活检组织病理学检查，并采用荧光原位杂交（fluorescence in situ hybridization，FISH）方法检测HER-2表达情况。结果36例取材满意率为100％，穿刺活检后无明显并发症发生；组织病理诊断浸润性癌26例，腺癌7例，导管原位癌1例，浸润性癌伴鳞状化生1例，大部分中一高级别导管原位癌＋小部分浸润性癌Ⅱ～Ⅲ级1例；HER-2阳性18例，阴性16例，2例无法确定。结论超声引导下粗针穿刺组织病理活检明确乳腺癌诊断，可作为乳腺癌HER-2基因检测的取材方法。Objective To study the value of ultrasound guided coarse needle biopsy to the diagnosis of breast cancer and the detection of human epidermal growth factor receptor-2 （HER-2）. Methods Thirty-six patients with breast cancer underwent ultrasound guided coarse needle biopsy, and fluorescence in situ hybridization was adopted to detect the expression of HER-2. Results Ultrasound guided coarse needle biopsy was successfully performed in 36 patients. No complications occurred after biopsy. Histoppathological results confirmed invasive carcinoma in 26 patients, adenocarcinoma in 7, ductal carcinoma in situ in 1, invasive carcinoma complicated with squamous metaplasia in 1, and middle and high grade DCIS in most part plus grade Ⅱ to Ⅱ invasive carcinoma in small part in 1 case. HER-2 was positive in 18 patients, negative in 16, and not determined in 2. Conclusion Ultrasound guided coarse needle biopsy can be used as the sampling method for the diagnosis of breast cancer and the detection of HER-2 gene.
