复方皂矾丸对晚期食管癌患者同步放化疗致骨髓抑制的保护作用

摘　　要：目的观察复方皂矾丸对晚期食管癌同步放化疗患者骨髓抑制的保护作用。方法将80例晚期食管癌施行同步放化疗患者随机分为治疗组与对照组各40例,2组均给予CF化疗+三维适形调强放疗治疗,治疗组加用复方皂矾丸1.8 g口服,3次/d。观察周期均为12周。观察2组治疗期间骨髓抑制情况及生活质量QLQ-C30(V3.0)量表评分变化。结果治疗后治疗组白细胞、中性粒细胞、血小板下降程度均明显低于对照组(P均<0.05),且血红蛋白较治疗前无明显下降(P>0.05),对照组则较治疗前明显下降(P<0.05);2组治疗后骨髓抑制程度均大于0度,且以Ⅰ、Ⅱ度为主,治疗组骨髓抑制Ⅲ、Ⅳ度发生率明显低于对照组(P均<0.05);治疗后对照组QLQ-C30(V3.0)量表评分较治疗前明显降低(P<0.05),而治疗组较治疗前无明显降低(P>0.05),2组比较差异有统计学意义(P<0.05)。结论晚期食管癌同步放化疗患者联合应用复方皂矾丸对骨髓抑制有明显保护作用,且可有效改善生活质量水平,值得推广应用。Objective It is to observe the protective effect of compound Zaofan Pill（ CZFP） on bone marrow suppression induced by radiotherapy and chemotherapy in patients with advanced esophageal cancer. Methods 80 cases of advanced esophageal cancer underwent concurrent radiotherapy and chemotherapy were randomly divided into treatment group and control group with 40 cases in each,all patients were treated with CF chemotherapy plus three-dimensional conformal radiotherapy,the patients in the treatment group plus CZFP 1. 8 g for oral,3 times per day. The observation period was 12 weeks. The changes of bone marrow depression and quality of life QLQ- C30（ V3. 0） scale score of the two groups were observed. Results After treatment,the decrease degree of WBC,neutrophil and platelet in the treatment group were significantly lower than those in the control group（ all P 0. 05）,and the level of hemoglobin with little reduction in the treatment group（ P 0. 05）while with significantly reduction in the control group than before treatment（ P 0. 05）. The bone marrow suppression degree of the two groups were all more than 0 degrees,and degree Ⅰ and Ⅱ were major,the incidence rate of degree Ⅲ and Ⅳ of bone marrow suppression of the treatment group was significantly lower than that of the control group（ P 0. 05）. The QLQ-C30（ V3. 0） scores with little reduction in the treatment（ P 0. 05） while with marked reduction in the control group（ P 0. 05）,there was significant difference between the both groups（ P 0. 05）. Conclusion concurrent radiotherapy and chemotherapy combined with CZFP has obvious protective effect on bone marrow suppression in patients with advanced esophageal cancer,and can effectively improve the level of quality of life,it is worthy of popularization and application.
