超声检查对海南原住居民后循环缺血的诊断价值

摘　　要：目的探讨多普勒超声(TCD)检查对海南地区原住居民后循环缺血(PCI)的诊断价值。方法选800例PCI患者及100例健康者,采用TCD及CT血管成像(CTA)检查,探查双侧大脑后动脉(PCA)、双侧椎动脉(VA)、基底动脉(BA)的峰值血流速度(Vs)、平均血流速度(Vm)、舒张末期血流速度(Vd),记录动脉搏动指数(PI)及频谱形态变化特征。结果研究组TCD检查管腔闭塞32条,轻度狭窄272条,中度狭窄96条,重度狭窄16条;双侧VA、PCA、BA的PI值及右侧VA、BA的Vs值显著高于对照组,双侧PCA的Vd值显著低于对照组(P<0.05);血流速度加快和减慢所占比例均显著高于对照组(P<0.05)。结论 TCD与CTA具有高度一致性,TCD检查能为PCI的诊断、病情评估、治疗和预后提供依据。Objective To explore the diagnostic value of transcranial Doppler （TCD） in the diagnosis of posterior circulation is- ehemia （ PCI ） in Hainan area residents. Methods A total of 800 cases of PCI and 100 healthy people were selected, who were examined by TCD and CT angiography （CTA） methods, and detected posterior cerebral artery （PCA ）, vertebral artery （ VA）, basilar artery（ BA） systolic velocity（ Vs）, mean velocity（ Vm）, diastolic velocity（ Vd）, pulsatility index（PI） and spectral mor- phological characteristics were recorded in two groups. Results TCD examination of the research group with 32 cases of lumen occlusion, 272 cases of mild stenosis, 96 cases of moderate stenosis, severe stenosis with 16 cases; Bilateral VA, PCA and Ba PI values, Vs values of right side VA and BA in research group were significantly higher than those in control group, Vd values of bilateral PCA were significantly lower than those in the control group （P〈0.05）. The blood flow velocity increase rate and blood flow velocity decrease rate was significantly higher than that in control group （ P〈0.05）. Conclusion TCD and CTA with high consistency; blood flow velocity. TCD method could provide the basis for the diagnosis, evaluation, treatment and prognosis of PCI.
