尿生存素联合液基细胞学检测对膀胱癌的诊断价值

摘　　要：目的膀胱癌是最常见的泌尿系恶性肿瘤,但关于如何提高它的诊断率的研究较少.文中旨在探讨采用尿生存素、液基细胞学联合检验提高膀胱尿路上皮癌的诊断率.方法收集2 0 1 2年1 月至2 0 1 4年1 2 月镇江市第一人民医院泌尿外科4 8 例膀胱癌患者为试验组,另收集4 8 例正常人尿液为对照组.取所有研究对象晨尿,进行尿液生存素检测和液基细胞学检验,应用R0 C 曲线分析上述单种检测方法及联合检验对膀胱癌诊断的临床价值.结果试验组中尿液生存素检测3 0 例诊断为膀胱癌（真阳性）,对照组中4 例诊断为膀胱癌（假阳性）.联合检测敏感度（9 1 . 7 % ）较尿生存素检测、液基细胞学检验（62.5%、60.4%）显著升高（P 〈0.05）.尿生存素检测、尿液基细胞学检验曲线下面积[0.771（0.673-0.868）、0.781（0.685-0.877）]较联合检测[0.906（0.839-0.974）]显著降低（P 〈0.05）. 结论尿生存素检测联合尿液基细胞学检测可以显著提高膀胱癌的检出率.Objective Bladder carcinoma is the most common urological malignancy,however there is less study on how to improve its diagnostic rate. The study was to improve the diagnosis of bladder urothelial carcinoma by combined detection of urinary surviving and liquid-based cytology. Methods 48 cases of bladder cancer patients were chosen as the experiment group,while urine samples of 48 normal people were collected as the control group. Combined detection of urinary surviving and liquid-based cytology was done on the morning urine of all subjects. ROC analysis was applied to analyze the clinical value of single detection and combined detection in bladder cancer. Results There were 30 cases of bladder cancer( true positive) in experiment group by urinary survivin detection and 4 cases of bladder cancer( false positive) in control group. The sensitivity of combined detection( 90.6%) significantly increased in comparison to that of urinary survivin detection( 62.5%) and urine-based cytology detection( 60.4%). The area under the curve( AUC) of urinary survivin detection( 0.771) and urine-based cytology detection( 0.781) decreased significantly in comparison to that of combined detection( 0.906). Conclusion combined detection of urinary surviving and liquid-based cytology can significantly improve the detection rate of bladder cancer.
