氟比洛芬酯复合利多卡因用于老年患者乳腺癌改良根治术的临床研究

摘　　要：目的探讨氟比洛芬酯复合利多卡因用于老年患者乳腺癌改良根治术(MRM)的安全性与可行性。方法52例老年患者随机分为2组。对照组患者在常规气管内插管全身麻醉后手术,观察组在对照组基础上于手术切皮前及手术结束前10 min静脉注射氟比洛芬酯1 mg·kg-1;术后对照组以舒芬太尼100μg和托烷司琼5 mg加生理盐水配成100 m L于镇痛泵中静脉自控镇痛,观察组以舒芬太尼75μg、托烷司琼5 mg和氟比洛芬酯100 mg加生理盐水配成100 m L于镇痛泵中静脉自控镇痛。观察手术过程中两组患者心率、平均动脉压、血氧饱和度及呼吸频率等生命体征的变化;记录两组患者术后0、4、8、12、24、48 h的VAS评分和不良反应发生情况。结果观察组患者在术后0、4、8、12、24 h各时间点时的VAS评分明显低于对照组(P<0.05);且在术后48 h内出现恶心、呕吐及注射部位疼痛等不良反应的发生率明显低于对照组(P<0.05)。结论氟比洛芬酯复合利多卡因在老年患者乳腺癌改良根治术中及术后的镇痛效果好,不良反应少,值得临床推广和应用。Objective To investigate the safety and feasibility of flurbiprofen axetil combined with lidocaine in elderlypatients of modified radical mastectomy （ MRM ） .Methods 52 cases of elderly patients that underwent MRM were randomlydivided into two groups. All patients were received general anesthesia during operation.The patients in observation group re-ceived flurbiprofen axeil 1 mg · kg-1. 10 minutes before incision and after operations respectively. After the operations,patients in control group were received patient - controlled intravenous analgesia （ PCIA ） , which containing 100 p.gsufentanil and 5 mg tropisetron in a volume of 100 mL; while the PCIA in observation group was containing 75 p.g sufen-tanil, 5 mg tropisetron and 100 mg flurbiprofen axetil in a volume of 100mL.The patient＇s reactive symptoms were observedand the vital signs were recorded. The degree of post-operative pain was accessed with the visual analogue scale{ VAS） atextubation time, 4 h, 8 h, 12 h, 24 h and 48 h after surgeries. The safety and adverse drug reaction of the method was alsoevaluated.Results The VAS scores of observation group at extubation time, 4 h, 8 h, 12 h and 24 h after surgery weresignificantly lower than control group （ P〈0.05 ） ; and the incidence of adverse reaction in observation group was significant-ly lower than control group （P〈0.05） .Conclusion Flurbiprofen axetil combined with lidocaine in elderly patients of MRMshowed good sedative effect and high safety.
