不同剂量^(131)I对分化型甲状腺癌患者血清中TH水平及生活质量的影响

摘　　要：目的:探讨不同剂量^(131)I对分化型甲状腺癌患者血清中甲状腺激素水平及生活质量的影响。方法:回顾性分析2012年1月—2015年3月我院收治的85例分化型甲状腺癌患者,所有患者均进行甲状腺癌切除手术,术后给予^(131)I治疗,根据剂量不同分为3组,A组为1次应用^(131)I治疗的患者,共27例,累计剂量为2.96~3.70 GBq,B组为2次应用^(131)I治疗的患者,共23例,累计剂量为8.14~10.36 GBq,C组3次应用^(131)I治疗的患者,共20例,累计剂量为12.95~17.02 GBq,比较3组患者治疗前后的临床疗效,生活质量评分,另15例同期未接受^(131)I治疗的患者作为对照。结果:^(131)I治疗组的有效率均显著高于对照组,B、C组有效率最高,A组次之(P<0.05);3组患者治疗后躯体功能、角色功能、认知功能、情绪功能以及社会功能等方面的评分均显著高于治疗前,B组治疗后的生存质量评分最高,C组次之,A组最低,组间差异有统计学意义。结论:^(131)I治疗分化型甲状腺癌疗效显著,可提高患者的生活质量,其中以B组的治疗方案效果最佳。Objective： To discuss the different doses of ^131I on serum thyroid hormone （TH） level and quality of life in the treatment of patients with differentiated thyroid cancer. Methods： Eighty-five patients with differentiated thyroid cancer were selected and underwent thyroid cancer resection surgery, and were postoperatively treated with ^131I. They were divided into group A （27 cases, treated with a cumulative dose of 2.96 - 3.70 GBq once）, B （23 cases, treated with a cumulative dose of 8.14 - 10.36 GBq twice） and C （20 cases, treated with a cumulative dose of 12.95 - 17.02 GBq for three times）. Their clinical efficacy and the scores of the quality of life were compared before and after treatment. Meanwhile, another fifteen patients who did not received ^131I treatment at the same time were selected as a control group. Results： The total effective rates were significantly higher in group A, B and C than in the control group with group B and C the highest （P〈0.05）. The scores of physical, role, cognitive, emotional and social functions were significantly higher after treatment than before and the scores of life quality were the highest in group B following by group C and A （P〈0.05）. Conclusion： The treatment of differentiated thyroid cancer with ^131I can significantly improve the quality of life of patients and possesses a clearly curative effect, and the therapeutic regimen in group B is the best.
