艾灸联合氟西汀治疗卒中后抑郁的临床观察

摘　　要：目的：观察艾灸联合氟西汀治疗脑卒中后抑郁（PSD）的疗效，并初步探讨其作用机制。方法脑卒中后抑郁患者56例随机分为观察组29例和对照组27例。观察组采用艾灸百会穴联合氟西汀治疗，对照组单纯采用氟西汀治疗。治疗前后分别对两组进行汉密尔顿抑郁量表（HAMD）评分，检测血浆5-羟色胺（5-HT）和去甲肾上腺素（NE）水平。结果观察组总有效率96.55%，对照组总有效率74.07%，差异有统计学意义（P〈0.05）；两组治疗8周后，观察组HAMD量表评分较对照组明显下降，差异有统计学意义（P〈0.01），血浆5-HT、NE水平较对照组明显升高，差异有统计学意义（P〈0.05）。结论艾灸联合氟西汀治疗脑卒中后抑郁的效果，优于单纯应用氟西汀；对调整脑卒中后抑郁患者体内单胺类神经递质的代谢水平，艾灸联合氟西汀治疗优于单纯氟西汀治疗。Objective To observe the clinical effect of moxibustion combined with fluoxetine on post-stroke depression,and to explore its mechanism. Methods 56 patients with post-stroke depression were randomly divided into two groups,the observer group was given moxibustion combined with fluoxetine,and the control group was given fluoxetine. The degree of depression were evaluated by HADM in pre-treatment and eight weeks after treatment;the concentration of noradrenergic （NE）and 5-hydroxy tryptamine（5-HT）in plasma were detected in pre-treatment and eight weeks after treatment. Results The total effective rate of the observer group was 96.55%,while the total effective rate of the control group was 74.07%. The difference was statistically significant（P〈0.05）. The HAMD scale scores of the observer group were significantly lower than the control group after 8 weeks of treatment（P〈0.01）,the concentration of noradrenergic （NE）and 5-hydroxy tryptamine（5-HT）in plasma were significantly higher than the control group（P〈0.05）. Conclusion The treatment effect of moxibustion combined with fluoxetine on Post-stroke depression is better than the simple application of fluoxetine. Compared with treated simply by fluoxetine,treating by moxibustion combined with fluoxetine can better adjustment for monoamine neurotransmitter metabolism of post-stroke depression patients.
