正畸治疗对唇腭裂术后牙颌面畸形患者正颌手术效果的影响研究

摘　　要：目的研究正畸治疗对唇腭裂术后牙颌面畸形患者正颌手术效果的影响。方法将2012年10月至2014年10月南京军区南京总医院收治的68例唇腭裂术后牙颌面畸形患者随机分为联合组和对照组,每组各34例。对照组患者仅采用正颌手术,联合组患者采用术前正畸治疗+正颌手术+术后正畸治疗。术后正畸于正颌手术结束4周后进行。比较两组患者的治疗效果及正颌术后并发症发生情况。结果正颌术后4周,两组患者SNA角、SNB角、ANB角与正颌术前比较均具有显著差异(P<0.01),但组间比较无显著差异(P>0.05)。正颌术后12个月,联合组患者SNA角、SNB角、ANB角与对照组比较均具有显著差异(P<0.01);正颌术后4周内,联合组患者并发症发生率(29.41%)明显低于对照组(58.82%)(P<0.05),且两组患者咀嚼功能评分和语言功能评分均明显高于正颌术前(P<0.01)。正颌术后12个月,联合组患者咀嚼功能评分和语言功能评分均明显高于对照组(P<0.01)。结论对于唇腭裂术后牙颌面畸形患者,在正颌手术基础上联合正畸治疗,有助于提高手术效果,减少术后畸形复发,降低术后并发症发生率,改善患者术后咀嚼和语言功能。Objective To research the influence of orthodontic treatment on orthognathic surgery for the dento-maxillofacial deformities patients with cleft lip and palate repairment. Method From October 2012 to October 2014, 68 cases of patients who had dento-maxillofacial deformities after cleft lip and palate repairmen were treated in our hospital. Randomly divided these patients into combination group and control group, 34 cases in each group. Patients in control group were treated with orthognathic surgery, patients in combination group were treated with the operation plan which included preoperative orthodontic surgery, orthognathic surgery and postoperative orthodontic treatment. Postoperative orthodontic treatment were applied 4 weeks after orthognathic surgery. The clinical effect and complications after orthognathic surgery between the two groups were compared. Result Compared with before orthognathic surgery, SNA angle, SNB angle and ANB angle of the two groups were significantly different 4 weeks after orthognathic surgery （P 〈 0.01）, but there were no significant differences between the two groups （P 〉 0.05）; 12 months after orthognathic surgery, two groups had obvious differences in SNA angle, SNB angle and ANB angle （P 〈 0.01）; the incidence rate of complications during 4 weeks after orthognathic surgery in combination group was 29.41%, control group was 58.82%, there was significant difference between the two groups （P 〈 0.05）. Compared with the scores before orthognathic surgery, both groups had better results in the chewing function score and language function score which were tested in the same group after orthognathic surger （P 〈 0.01）; 12 months after orthognathic surgery, the chewing function score and language function score in combination group were both obviously more than those in control group （P 〈 0.01）.
