喹硫平两种剂量快速加量治疗精神分裂症急性期患者的效果比较

摘　　要：目的探讨比较喹硫平两种剂量快速加量对精神分裂症急性期患者的临床治疗效果。方法将2014年1月至2015年12月入院的60例精神分裂症急性期患者随机分为两组,A组患者以100 mg/d作为起始剂量,7 d内升至600 mg/d,B组则以200 mg/d作为起始剂量,4 d内升至600 mg/d,比较两组患者治疗前后精神症状、生活质量、总体疗效及治疗期间不良反应情况。结果治疗后,两组患者的PANSS评分与SQLS总分均显著低于治疗前,差异具有统计学意义(P<0.01);两组患者治疗后的PANSS与SQLS总分比较无统计学意义(P>0.05)。两组患者的临床总有效率比较,差异无统计学意义(P>0.05);A组患者不良反应发生率(30.00%)显著低于B组(63.33%),差异具有统计学意义(P<0.01)。结论两种剂量喹硫平快速加量对精神分裂症急性期患者均有显著疗效,但以100 mg/d为起始剂量,7 d内升至600 mg/d的剂量治疗期间不良反应更少,安全性更高,具有借鉴性。Objective To explore and compare the clinical therapeutic effect of fast increasing quantity of two kinds of dose of quetiapine in the treatment of patients with acute phase of schizophrenia. Methods Sixty cases of schizophrenia disease at acute stage from January 2014 to December 2015 in our hospital were randomly divided into two groups. Group A were treated with 100 mg/d as a starting dose, and the dose rosed to 600 mg/d within 7 days. Group B were given 200 mg/d as a starting dose, and the dose rosed to 600 mg/d within 4 days. Psychiatric symptoms, quality of life, overall curative effect and treatment during adverse reactions were compared between two groups of patients before and after treatment. Results After treatment the PANSS and SQLS scores of two groups had statistical significance from those of before treatment （P〈0.01）, while the above scores had no significant difference between the two groups after treatment. The overall curative effect existed no significant difference between the two groups （P〉0.05）. The overall incidence of adverse reactions of group A （30.00%） was significantly lower than that of group B （63.33%）, the difference was significant. Conclusion Both the two kinds dose of fast increasing quantity of quetiapine showed significant treatment effects, but the treatment of 100 mg/d as a starting dose, and the dose rose to 600 mg/d within 7 days has less adverse reaction, better security, which has reference for clinic.
