中青年与老年结肠癌患者临床病理特点比较分析

摘　　要：目的:分析中青年与老年结肠癌患者临床病理特点,为不同年龄段结肠癌患者早期诊断、早期治疗提供依据。方法:选取笔者所在医院收治的56例中青年结肠癌患者(≤50岁)及72例老年结肠癌患者(≥60岁)作为研究对象,整理所有患者的病例资料,对两组结肠癌患者的临床病理特点进行对比分析。结果:两组结肠癌患者相同特点为肿瘤好发部位以升结肠和乙状结肠为主,病理类型以隆起管状腺癌、溃疡型腺癌多见,中青年组与老年组患者远期生存率比较差异无统计学意义(P>0.05)。中青年组临床症状缺乏特异性,组织学类型以中、低分化为主,Dukes分期中青年组C、D期患者比例明显高于老年组,差异有统计意义(P<0.05)。结论:中青年结肠癌患者与老年结肠癌患者临床症状无明显区别,警惕性症状均少见,因此多注意排便习惯、性状改变并普及结肠镜等相关检查对于中青年及老年结肠癌早期诊断和预后具有重要意义。Objective：To analysis the clinical pathological characteristics of young and middle-aged and elderly patients with colon cancer,to provide the basis for early diagnosis and early treatment for different age patients with colon cancer.Method： 56 cases of young and middle-aged patients with colon cancer（50 years old）and 72 cases of elderly patients with colon cancer（60 years old） in the author＇s hospital were selected as the research object,the pathological data of all patients were settled,the clinical pathological characteristics of two groups of patients with colon cancer were analyzed.Result：The Common characteristics of two groups of patients with colon cancer was that the tumor location was ascending colon and sigmoid colon,the mainly pathologic types were uplift tubular adenocarcinoma and ulcer type adenocarcinoma,the long-term survival rate of young and middle-aged and elderly patients were no significant difference（P〉0.05）.Clinical symptoms of Young and middle-aged group were lack of specificity,and histological type were middle and low differentiation mainly,Dukes stage C and D proportion of patients in young and middle-aged group was significantly higher than that of patients in elderly group,the difference was statistically significant（P〈0.05）.Conclusion：Clinical symptoms in young and middle-aged patients with colon cancer and elderly patients has no obvious difference,vigilance symptoms are rare,so paying more attention to bowel habit,and nature change and popularization of colonoscopy for young and middle-aged and elderly colorectal cancer early diagnosis and prognosis has important significance.
