240例儿童癫痫临床疗效影响因素的分析

摘　　要：目的探讨不同临床类型儿童癫痫的发作控制情况,为评价儿童癫痫病例的预后提供初步参考。方法总结延安市人民医院儿科2011~2014年确诊的240例癫痫患儿,分析其临床发作类型、脑电图（EEG）特点、智能发育水平及发作控制情况。结果 240例癫痫患儿中,临床局灶性发作178例（74.17%）,总治疗有效率为75.00%。在所有病例中,伴中央颞区棘波的儿童良性癫痫（BECTS）及失神发作的控制情况较好,其治疗有效率分别为92.59%、88.89%;而婴儿痉挛的总有效率仅为22.22%。在局灶性发作患儿中,智能发育落后者较智力发育正常者的疗效差,EEG背景节律变慢或失节律者较EEG正常者的疗效差（χ~2值分别为10.16,14.52;均P〈0.01）。结论儿童癫痫发作以局灶性发作为主,其临床疗效与癫痫类型、智能发育水平及EEG特点等关系密切。Objectve To investigate the control of epilepsy seizures in children with different clinical types and to provide preliminary evaluation reference for prognosis of childhood epilepsy. Methods A total of 240 cases diagnosed with epilepsy in pediatric department of Yan＇an Peoples Hospital from 2011 to 2014 were summarized. The clinical seizures type,electroencephalogram（ EEG） features,intelligent developmental level and control of seizures were analyzed. Results Among 240 cases of epilepsy,there were 178 cases（ 74. 17%） of clinical focal seizures. And the total effective rate was 75. 00%. In all cases,BECTS and absentia seizures had better control,of which the treatment efficiency was 92. 59% and 88. 89%,respectively. The total effective rate of infantile spasms was only22. 22%. In cases of focal seizures in children,those with mental development laggard had poorer efficacy compared to those with normal mental development,and those with EEG background with slow or lost rhythm had poorer efficacy compared to those with normal EEG（ χ~2value was 10. 16 and 14. 52,respectively,both P〈0. 01）. Conclusion Focal seizure is the main type of childhood epilepsy seizure,and its clinical efficacy is closely related with epilepsy types,mental development level and EEG characteristics.
