摘 要:目的探讨p53、p16、PTEN、Ki-67、p63在子宫内膜腺癌组织中的表达及其与临床预后的相关性,并分析这些基因在腺癌中表达间的相关性。方法采用免疫组化S-P法检测2001年1月至2015年11月西京医院收治的782例子宫内膜腺癌患者的癌组织石蜡标本中p53、p16、PTEN、Ki-67、p63的蛋白表达情况。结果 1p53、p16、PTEN、Ki-67、p63在子宫内膜腺癌组织中的阳性表达率分别为61.9%,76.2%,10.9%,54.2%,19.6%;2p53与临床分期、组织分化、脉管瘤栓形成、死亡有关,均P<0.000;3p16与组织分化、脉管瘤栓形成、复发及死亡有关,P值分别为:0.003、0.023、0.000、0.000;4PTEN与组织分化、脉管瘤栓形成、死亡有关,P值分别为:0.000、0.000、0.007;5Ki-67与临床分期、肌层浸润、组织分化、复发及死亡有关,P值分别为:0.000、0.000、0.043、0.000;6p63与组织分化、脉管瘤栓形成、复发有关,P值分别为:0.000、0.000、0.042(均P<0.05);7p53与p16、Ki-67、p63的表达呈正相关,相关系数分别为0.087、0.103、0.128(均P<0.05);8p16与PTEN、Ki-67、p63的表达呈正相关,相关系数分别为0.079、0.071、0.245(均P<0.05);9PTEN与p63的表达呈正相关,相关系数为0.242(P<0.05)。结论 p53、p16、PTEN、Ki-67、P63基因在子宫内膜腺癌中的表达存在明显相关性,它们在子宫内膜癌的发生、进展及影响其预后中可能起协同促进作用,联合检测这些基因表达可能更确切地反应肿瘤进展的动态改变,对判断子宫内膜腺癌恶性程度及评估预后有一定的参考价值。Objective To investigate the expressions of p53, p16, FFEN, Ki-67 and p63 in endometrial adenocarcinoma (EA) and their correlation with clinical prognosis, and to analyze the correlation among expressions of these genes. Methods Totally 782 cases of EA who sought medical advice at Xijing Hospital during the period of 2001 -2015 were included. Immunohistochemical SP method was applied to detect the expression of paraffin specimens in cancer tissues of these cases. Results The positive expression rates of p53, p16, PTEN, Ki- 67 and p63 were 61.9%, 76.2%, 10.9% , 54.2% and 19.6%, respectively in EA. The expression of p53 was correlated with clinical stage, histologic classification, vascular tumor thrombus and death ( all P 〈 0. 000 ) , and the expression of p16 was correlated with histologic classification, vascular tumor thrombus, recurrence and death (P value was 0. 003, 0.023, 0. 000 and 0.000, respectively). The expression of PTEN was correlated with histologic classification, vascular tumor thrombus and death ( P value was 0.000, 0.000 and 0. 007, respectively), and the expression of Ki-67 was related to clinical stage, muscle invasion, histologic classification, recurrence and death (P value was 0. 000, 0. 000, 0. 043 and 0. 000, respectively). The expression of p63 was related to histologic classification, vascular tumor thrombus and recurrence ( P value was 0. 000, 0. 000 and 0. 042, respectively) ( all. P 〈0.05 ). The expression of 1353 was positively correlated with the expressions of p16, Ki-67 and 1363, and the coefficient was 0. 087,0. 103 and 0. 128, respectively( all P 〈 0.05 ). The expression of p16 was positively correlated with PTEN, Ki-67 and p63, and the coefficient was 0.079, 0.071 and 0.245 (P 〈 0.05). PTEN was positively related to p63 in expression with the coefficient of 0.242 (P 〈 0.05). Conclusion Expressions of p53, p16, PTEN, Ki-67 and p63 are significantly correlated in EA. They have significantly interactive impacts on the development
