摘　　要：目的评价怀旧疗法(RT)对轻、中度痴呆患者行为和精神症状(BPSD)的作用。方法采用随机对照研究,收集符合标准的轻、中度痴呆患者51例,随机分为对照组(25例)和研究组(26例),对照组给予常规药物治疗,研究组在药物治疗基础上联合怀旧疗法,两组分别于治疗前、治疗8周末、治疗结束后4周(随访)3个时间点采用神经精神科问卷(NPI)评估行为和精神症状。结果经过重复测量方差分析,两组NPI总分在分组主效应(F=4.614,P=0.037)、时间主效应(F=100.100,P=0.000)、分组×时间的交互效应(F=26.115,P=0.000)的差异均具有统计学意义。因子分析中攻击/激越、异常的运动行为2个因子的分组主效应、时间主效应、分组×时间的交互效应具有统计学差异(P<0.05),抑郁/恶劣心境因子在分组主效应、时间主效应的差异具有统计学意义(P<0.05)。结论怀旧疗法可显著减轻痴呆患者的行为和精神症状,值得临床实践推广。Objective To explore the effect of reminiscence therapy （RT） on behavioral and psychological symptoms （BPSD） in patients with mild and moderate dementia. Methods We conducted a randomized, controlled trial. A total of 51 Dementia patients were randomly divided into the control group （n=25） and the study group （n=26）. Each case of the control group was treated by drug therapy, while the control group received RT combined with drug treatment. The Neuropsychiatric Inventory （NPI） was used to assess the BPSD before treatment, 8weeks after the treatment, and 4 weeks after follow-up. Results The difference between the two groups in NPI scores and the main effect （F=4.614, P =0.037）, time main effect （F=100.100, P=0.000）, the interaction effect of group×time （F=26.115, P=0.000） were significant. In addition, 12 factors were analyzed which showed that the agitation, aberrabt motor behavior were significantly different in group main effect, time main effect and the interaction effect of group×time （P〈0.05）, while the factor of dysphoria was significantly different in main effect of group and time （P〈0.05）. Conclusions The reminiscence therapy can significantly reduce the behavior and mental symptoms of patients with dementia, and it is worth to be popularized.
