摘  要:目的:分析中枢神经系统(CNS)孤立性纤维瘤(SFT)的临床及影像学表现。方法:回顾性分析22例CNS中SFT的CT和MRI表现及临床特征。结果:病灶均呈实性边界清晰的占位性病变,直径12~70 mm,平均38 mm;病灶形态呈类圆形16例(72.7%),不规则形6例(27.3%);10例经平扫CT显示,9例呈高密度,1例呈低密度,2例伴有骨质侵犯,肿块均无明显钙化;18例MRI扫描显示:T1WI呈低信号5例,等信号7例,不均匀等低混杂信号6例,而T2WI呈等信号13例,低信号4例,高信号伴实质内散在低信号1例;增强扫描后呈明显均匀强化10例,明显不均匀强化12例;14例邻近脑膜增厚。结论:虽然SFT是CNS少见肿瘤,但仍具有一定的特征性表现,如常发生于桥小脑角区、CT呈高密度、MRI呈等低信号并明显强化,通过分析总结其临床及影像学特征,可提高对该病的诊断。Objective:To analysis clinical and imaging of the solitary fibrous tumors(SFT) in central nervous systerm(CNS).Method:The MRI and CT clinical features of SFT in 22 cases of CNS were retrospectively analyzed.Result:The lesions showed a solid and clear space occupying lesion,diameter was 12-70 mm,averaged 38 mm.The shape of the lesion was round in 16 cases(72.7%) and 6 cases(27.3%) were irregular.10 cases showed a high density of 9 cases,1 cases showed low density,2 cases with bone invasion and no obvious calcification in all cases.18 cases MRI scan showed that 5 cases of T1 WI low signal,7 cases of isointense signal,6 cases of isointense low mixed signal,13 cases of T2 WI isointense signal,4 cases of low signal,1 case of high signal with the essence of scattered low signal.After the enhancement,10 cases were obvious homogeneous enhancement and 12 cases were obviously inhomogeneous enhancement.14 cases of adjacent dural thickening.Conclusion:Although SFT is rarely seen in CNS,but it still has certain characteristic performance,including mostly find in cerebellopontine angle zone,hyperattenuation on CT and hypointense signal on MRI images and obvious enhancement,the diagnostic acurracy could be improved by summarising the clinical and imaging features of this tumor.
