盐酸羟考酮注射液用于食管癌根治术术后镇痛的临床效果

摘　　要：[目的]观察盐酸羟考酮注射液在食管癌根治术术后病人自控静脉镇痛（PCIA）中的效果。[方法]选取本院诊治的食管癌根治术患者80例，根据术后PCIA镇痛药物不同分为对照组40例和观察组40例。对照组采用硫酸吗啡＋托烷司琼混合生理盐水镇痛，观察组采用盐酸羟考酮注射液＋托烷司琼混合生理盐水镇痛，比较两组术后镇痛效果及不良反应发生率。[结果]两组患者术后5min血压、心率水平相比较差异无显著性（P〉0．05）；观察组患者术后30min血压低于对照组，心率高于对照组，且两组相比较差异有显著性（P〈0．05）；观察组患者术后2h、4h、12h视觉模拟评分（VAS）显著低于对照组（P〈0．05）；观察组患者术后苏醒时间显著短于对照组（P〈0．05）；观察组患者术后不良反应发生率为7．5％（3／40）显著低于对照组的22．5％（9／40）（P〈0．05）。[结论]食管癌根治术术后患者采用盐酸羟考酮注射液＋托烷司琼混合生理盐水PCIA镇痛效果理想，不良反应发生率低，值得推广应用。[Objective]To observe the effect of oxycodone hydrochloride injection on postoperative analgesia after esophageal carcinoma operation. [Methods]A total of 80 patients at our hospital for surgery as treatment of esophageal cancer surgery were divided according to postoperative analgesic drugs used into a control group of 40 patients and an observation group of 40 cases. The control group received morphine sulfate ＋ tropisetron saline mixed analgesic, while the observation group received oxycodone hydrochloride injection in addition to the tropisetron ＋ saline mixed analgesic. The postoperative analgesia and incidence of adverse reactions were compared between the two groups. [Results] The two groups showed no difference in blood pressure and heart rate level 5 min post-operation, but after 30 min, blood pressure was lower in the observation group than in the control group, while heart rate level was higher （ P〈0.05）. VAS （Visual Analog Score） of patients in the observation group 2h, 4h, 12h was significantly lower than that of the control group （ P 〈0.05）. The recovery time of patients in the observation group was significantly shorter than that of patients in the control group （ P 〈0.05）. The rate of occurrence of postoperative adverse events in the observa- tion group was 7.5 %, significantly lower than the rate of 22.5 % in the control group （ P 〈0.05）. [Conclusion]Postoperative esophageal carcinoma treated with oxycodone bydrochloride injection ＋ tropisetron over saline mixed had good analgesic effect and results in a low incidence of adverse reactions. It should be widely applied.
