《伤寒论》脾约证证治探析

摘　　要：太阳阳明是脾约,正阳阳明是胃家实,脾约证的病位在脾,而不是在胃;脾约证是介于承气汤证与蜜煎导证之间的一种虚实夹杂的病证,本证之虚为脾阴亏虚,而非脾气亏虚,病机为阳盛阴受约。脾胃互为表里,脾主为胃行津液,生理上互相配合,病理上相互影响。脾约证之所成,乃由脾胃之气亢盛,过量吸收胃中津液,上输于肺,通调水道,下输膀胱,而小便数;阳盛而阴虚,脾胃中之津液亏虚,肠腑干涸,濡润失职,而中消便难。故本证实为虚实错杂之证,治疗当以麻子仁丸缓攻胃肠之燥热,滋养脾胃之阴津。Taiyang yangming is constipation of splenic origin, while zhengyang yangming is gastrointestinal excess syndrome. The disease location of ＂syndrome of constipation of splenic origin＂ lies in the spleen, not in the stomach. ＂Syndrome of constipation of splenic origin＂ is an intermingled syndrnme of deficiency and excess between syndrome of Purgative Decoction and syndrome of Mijiandao （MJD）. its deficiency is deficient splenic yin rather than deficiency of spleen,and its pathogenesis is excessive yang influencing yin. The spleen and stomach has an internal-external relation. The spleen controls digestion while the stomach generates fluid. Physiologieally they interact with each other, while pathologically they influence each other. The etiolngy of ＂syndrome of constipation of splenic origin＂ is that excessive spleen-stomach qi absorbs stomach fluid excessively,sends up to the lung, regulates the water passage,sends downs to the bladder,sot there is much urination;When excessive yang and deficient yin lead to insufficient fluid in the spleen and stomach,intestines will become dry failing to moisten,and the digestion of the stomach will become difficuh. So ＂ syndrome of cnnstipation of splenic origin＂ is an intermingled syndrome of deficiency and excess, Cannabis Fruit Pill shouht be used to slowly treat dryness and heat in the stomach and intestines, nourish fluid in the stomach and intestines.
