江苏省连云港市区2012年恶性肿瘤疾病负担分析

摘　　要：[目的]对2012年连云港市区恶性肿瘤疾病负担进行评价。[方法]根据连云港市肿瘤登记系统上报的数据,计算因肿瘤早死所致寿命损失年（YLLs）、残疾所致健康寿命损失年（YLDs）和伤残调整寿命年（DALYs）。[结果 ]2012年连云港市区恶性肿瘤死亡率为156.23/10万、中标率为128.50/10万。2012年恶性肿瘤所致的DALYs为24.78/千人,男性和女性分别为27.19/千人和22.27/千人;YLLs为20.52/千人,男性和女性分别为24.21/千人和16.67/千人;YLDs为4.26/千人,男性和女性分别为2.98/千人和5.60/千人。恶性肿瘤DALYs损失以早死所致寿命年损失为主。除乳腺癌等个别癌种外,早死所致寿命年损失男性均高于女性,而残疾所致寿命年损失女性高于男性。DALYs损失前5位顺位和死亡率顺位一致,依次为肺癌、肝癌、胃癌、食管癌和结直肠癌。DALYs在45~59岁年龄组最高。[结论]45岁以上的人群应当作为癌症预防的重中之重,肺癌、肝癌和乳腺癌应作为连云港市区肿瘤预防的重点。[Purpose] To estimate the disease burden of malignant tumor among the city residents in Lianyungang. [Methods] Based on data of ＂Lianyungang Cancer Registry System＂ ,years of life lost（YLLs）, years lived with disability （YLLs）, and disability adjusted life year （DALYs） were calcu- lated to investigate the disease burden of malignant tumor. [ Results ] The mortality and standard- ized mortality rate were 156.23/105 and 128.50/105 respectively. The DALYs of malignant tumor was 24.78 years per 1000 persons （27.19 for male and 22.27 for female） , while the YLLs were 20.52 years per 1000 persons （24.21 for male and 16.67 for female） and the YLDs were 4.26 years per 1000 persons （2.98 for nmle and 5.60 for female） respectively. The DALYs loss mostly due to YLLs. In addition to female breast cancer, the YLLs were higher in male than that in female. Lung cancer,liver cancer,stomach cancer,esophageal cancer and colorectal cancer were the top tumors of both mortality and DALYs. Most of the DALYs focus on the residents above 45 years old. [ Conclusions] The emphasis of cancer prevention should be pay attention on the people above 45 years old. Comprehensive control measures should be carried out for these malignancies with greater burden such as lung cancer, liver cancer and breast cancer.
