摘 要:目的:本实验通过观察局部应用中药提取物菟丝子多糖结合牵张成骨技术促进兔胫骨骨折断端的成骨情况,证实菟丝子多糖能够加速牵张成骨,并说明补肾中药促进成骨的部分机理。方法:选取成熟新西兰大白兔24只,体重(2.5±0.5)Kg,按随机原则分为菟丝子多糖组和盐水对照组。菟丝子组于术中和术后第2w骨折断端注射菟丝子多糖0.5ml(0.5ml:200mg),盐水对照组于术中和术后第2w骨折断端注射生理盐水0.5ml,2组均于术后7d开始延长,每天延长lmm,共延长10天,延长完成后再固定6w,统一处死各组所有实验动物,并留取完整标本。于术后不同时期(延迟期10d后1w、2w、3w、6w)的x线动态观察、Micro—CT扫描、生物力学检测等方面分析2组之间促进成骨有无差异。结果:①动态x线观察示:菟丝子多糖组在完成牵张后第2dd,即可以看出延长区成骨明显高于盐水组,表现出明显促进骨痂生成的作用;②Micro—CT扫描:菟丝子多糖组成骨量明显大于盐水对照组,且密度也明显增高。其中,TB.TH.3D检测值比较无显著差异,2组之间无统计学意义(D0.05)。BMC、BMD、TMC、TB.SP.3D检测值比较差异明显,有统计学意义(P〈0.05);③生物力学测试结果示:菟丝子多糖组与对照组的最大扭矩角(deg)无显著差异,没有统计学意义(D0.05);最大扭矩(N—m)和扭转刚度(N—m/deg)差异显著,有统计学意义(P〈0.05)。结论:在兔胫骨牵张成骨试验中,联合菟丝子多糖牵张成骨比单独应用外固定架牵张成骨在同一时间内成骨量更大、骨化更完全。即局部注射菟丝子多糖能加速促进牵张成骨,缩短治疗时间,具有很好的实用价值。Objective: To analysis the effect of cuscuta chinensis polysaccharide local injection combine with distraction osteogenesis in treatment of rabbit tibia bone defect. At the same time, to confirm that cuscuta chinensis polysaecharide can accelerate the process of bone formation, and to preliminary clarify the mechanisms of traditional Chinese medicine in promoting osteogenesis. Methods: 24 mature New Zealand white rabbits weighing (2.5±0.5)kg were randomly divided into cuscuta chinensis polysaccharide treatment group (group A) and saline injection control group (group B). Cuscuta chinensis polysaccharide 0.5ml (200mg) was locally injected into the end of fractured bone in group A intraoperatively and 2 weeks after operation respectively, and an equal volume of normal saline was given to group B at the same time. One week after operation, both groups to begin to lengthen, lmm daily, for 10 days in all, then fixed for 6 weeks. After that, all rabbits of 2 groups were killed and complete specimens were remained. 1, 2, 3, 6 weeks after 10 days extending period, X-ray dynamic observation, Micro-CT scan, biomechanical detection were respectively carried out to analyze the difference in osteogenesis between the 2 groups. Results: Dynamic X-ray showed that osteogenesis in the extension area in group A was higher than that in group B at the 24 days after stretched, which was displaying an obvious callus formation. Micro-CT scan showed the amount of bone formation in group A was more than that in group B, and the density was raised significantly. Among the total, there was no any significant difference in the value of TB.TH.3D (P〉0.05), but was a significant difference in the value of BMC, BMD, TMC, TB.SP.3D (P〈0.05) between 2 groups. Biomechanical test results showed there was a significant difference in the maximum torque(N-m) and torsional rigidity(N-m/deg) (P〈0.05), but no any significant difference in the maximum torque angle(deg) (P〉0.05), between group A and
