摘 要:目的观察他汀类药物治疗高血脂非增生型糖尿病视网膜病变(DR)的临床疗效。方法 60例高血脂非增生型DR患者随机分为常规治疗组、辛伐他汀组和阿托伐他汀组,每组20例。3组均给予低胆固醇饮食、运动指导、口服降糖药和(或)胰岛素强化等常规治疗,辛伐他汀组和阿托伐他汀组在此基础上分别加用辛伐他汀、阿托伐他汀。连续治疗1、6个月后,比较3组的总有效率及治疗前后患者的血糖、血脂、血浆黏度变化。结果治疗6个月后,阿托伐他汀组的疗效明显高于辛伐他汀组和常规治疗组(P<0.05或0.01);治疗1、6个月后,辛伐他汀组、阿托伐他汀组患者的TC、TG、LDL-C及血浆黏度水平均比治疗前明显降低,HDL-C明显增高,且以阿托伐他汀组和治疗6个月的变化更为显著(P<0.05或0.01);常规治疗组的TC、TG水平也比治疗前降低(P<0.01),LDL-C、HDL-C及血浆黏度则变化不明显。3组的血糖无明显改变。结论他汀类药物可有效改善高血脂非增生型DR患者的高血脂及微循环状态,且以阿托伐他汀效果更好。Objective To observe clinical efficacy of statins in the treatment of patients with hyperlipidemia and non- proliferative diabetic retinopathy. Methods 60 patients with hyperlipidemia and non-proliferative diabetic retinopathy admitted to our hospital from February 2014 to June 2016 were randomly divided into Control Group (routine treatment)20 cases, Simvastatin Group (routine treatment combined with simvastatin) and Atorvastatin Group (routine treatment combined with atorvastatin), 20 cases in each group. After a treatment for 1 month and 6 months, the total effective rate as well as glucose sugar, blood lipid and plasma viscosity were compared among three groups. Results After a treatment of 6 months, the total effective rate of Atorvastatin Group was higher than that of Sirnvastatin Group and Control Group (P〈0.05 or 0.01). After a treatment of 1 and 6 months, TC, TG, LDL-C and plasma viscosity of Atorvastatin Group and Simvastatin Group were significantly decreased compared with those before the treatment, and HDL-C of Atorvastatin Group and Simvastatin Group were significantly increased compared with those before the treatment. The changes were more significant in Atorvastatin Group and those receiving a treatment of 6 months (P〈0.05 or 0.01). TC and TG level in Control Group were decreased compared with those before the treatment (P〈0.01). However, the changes in LDL-C, HDL-C and plasma viscosity were insignificant. There was no significant change in blood glucose in the three groups. Conclusion Statins can effectively improve the hyperlipidemia and microcirculation status of the patients with hyperlipidemia and non-proliferative diabetic retinopathy, especially atorvastatin.
