摘  要:目的探讨双胎妊娠母体血中胎儿游离DNA的比例及其与妊娠周数和孕妇体重指数的关系。方法研究对象为2015-10-01—2016-08-31在中国医科大学附属盛京医院产科就诊,自愿接受无创产前检测的妊娠11~30周双胎孕妇。通过后续随访,确认已有妊娠结局的男-男双胎71例,其中单绒毛膜双羊膜囊双胎31例,双绒毛膜双羊膜囊双胎40例。应用高通量测序技术,根据Y染色体的浓度估算胎儿游离DNA占孕妇血浆中总DNA的比例。结果双胎妊娠孕妇血浆中胎儿游离DNA比例5%~30%,并与孕周呈正相关(相关系数r=0.71),与孕妇体重指数呈负相关(相关系数r=-0.78)。结论在11~30周双胎妊娠母体血浆中,胎儿游离DNA的比例占总DNA的5%~30%,并随着孕周的增加而上升,随着孕妇的体重指数增加而下降。Objective To explore the proportion of cell free fetal DNA in maternal plasma of twin pregnancies,and study the relationship between the proportion and gestational week and gravida body mass index.Methods Twin pregnant women at 11~30 weeks were recruited from the Obstetrics and Gynecology Clinic of Shengjing Hospital of China Medical University from September 1st 2015 to August 1st 2016.After signed consent,they accepted noninvasive prenatal testing by their own volition.The 31 MCDA and 40 DCDA male-male twins were analyzed after follow-up.Highthroughput sequencing technology was performed to estimate the proportion of cff DNA according to the concentration of fetal SRY-specific DNA.Results The fraction of cff DNA in maternal plasma in twin pregnancies with two male fetuses during 11~30 gestational weeks was about 5%~30%.The fraction had positive correlation with gestational week(correlation r=0.71)and negative correlation with gravid body mass index(correlation r=-0.78).Conclusion The fraction of cff-DNA in maternal plasma in twin gravidas is about 5%~30%,and increases with increasing gestational age,while decreases with the increasing pregnant women's body mass index.
