摘  要:目的探讨IL-17/NF-κBp65信号通路在老年患者急性脑出血发病及组织损伤中的作用。方法选择2012年7月至2015年7月宁夏回族自治区人民医院收治的58例急性脑出血老年患者作为脑出血(ICH组),按照发病至手术时间分为〈6h组(18例),6h-组(14例),25h-组(10例),49h-组(10例)和〉72h组(6例),ICH患者均行开颅血肿清除术;另选择同期医院行第三脑室造瘘术的12例阻塞性脑积水患者作为对照组。两组患者术中取病灶周围脑组织,免疫组化染色检查IL-17、NF-κBp65阳性表达情况,免疫印迹法检测IL-17、NF-κBp65蛋白表达,实时定量RT-PCR检测IL-17、NF-κBp65mRNA表达。结果 NF-κBp65主要在细胞核中表达,IL-17主要在细胞质中表达;对照组胶质细胞和神经元细胞少见IL-17、NF-κBp65表达,ICH组胶质细胞和神经元细胞可见大量IL-17、NF-κBp65表达,脑出血后随着时间的延长,IL-17、NF-κBp65阳性细胞数量呈先上升后降低趋势。ICH各组IL-17、NF-κBp65蛋白及mRNA表达水平均显著高于对照组(均P〈0.05);其中6h-组IL-17蛋白及mRNA表达最高,分别为(0.81±0.11)和(0.83±0.13);25h-组NF-κBp65蛋白及mRNA表达最高,分别为(0.77±0.12)和(0.81±0.11)。结论 IL-17/NF-κBp65信号通路参与调节老年急性脑出血后炎症反应,提示调控IL-17/NF-κBp65信号通路可能缓解急性脑出血后脑组织损伤。Objective To explore the mechanism of IL-17/NF-κB p65 signaling pathway in acute cerebral hemorrhage in elderly patients.Methods A total of 58 elderly patients with acute cerebral hemorrhage who were treated in People's Hospital of Ningxia Hui Autonomous Region from July 2012 to July 2015 were enrolled as intracerebral hemorrhage(ICH)group in the study.They were divided into 5groups according to the time duration from the onset of the disease to operation:less than 6h group(n=18),more than 6hgroup(n=14),more than 25 hgroup(n=10),more than 49h group(n=10)and more than 72 h group(n=6).ICH patients underwent craniotomy for evacuation of hematoma.Another 12 patients with obstructive hydrocephalus who underwent endoscopic third ventriculostomy served as control group.The brain tissues around the lesion were obtained during the operation.The positive expression of IL-17 and NF-κB p65 was detected by immunohistochemical staining and the protein levels by Western blotting.The mRNA expression of IL-17 and NF-κB p65 was detected by real-time quantitative RTPCR.Results NF-κBp65 was mainly expressed in the nucleus,and IL-17 in the cytoplasm.They were rarely expressed in glial cells and neurons in control group and were robustly expressed in ICH group.The number of IL-17-positive and NF-κB p65-positive cells was initially increased and then decreased with the extension of hemorrhage.The expression levels of IL-17 and NF-κB p65 protein and mRNA were significantly higher in each experimental group than those in the control group(all P〈0.05).The expression levels of IL-17 protein and mRNA were highest in more than 6h group[(0.81±0.11)and(0.83±0.13)respectively],and those of NF-κB p65 protein and mRNA were highest in more than 25h group[(0.77±0.12)and(0.81±0.11)respectively].Conclusion IL-17/NF-κB p65 signaling pathway participates in the regulation of inflammatory responses after acute cerebral hemorrhage,which indicates that regulation of IL-17/NF-κB p65 signaling p
