摘 要:目的:观察丙泊酚对脑损伤术后血浆β-内啡肽(β-EP)、血清热休克蛋白70(HSP70)、血乳酸及血糖水平的影响。方法:90例脑损伤开颅术后病人随机分成对照组和丙泊酚组,测定两组病人转入ICU即刻、转入后24、48以及72 h血浆β-EP值、血清HSP70及血乳酸值,并测定转入即刻至转入后72 h内每4 h的血糖值。结果:与对照组比较,转入后24、48和72 h丙泊酚组患者β-EP、HSP70和血乳酸值降低,差异有统计学意义(P<0.05);转入后第一个4 h,对照组患者血糖值(11.58±2.65)mmol/L高于丙泊酚组患者血糖值为(9.61±1.71)mmol/L,差异有统计学意义(P=0.024)。结论:脑损伤应激反应过程中应用丙泊酚,可降低相关应激指标的水平,增加机体对应激的调控。Objective:To observe the effects of propofol onβ-endorphin(β-EP),heat shock protein 70( HSP70 ),blood lactatic acid,blood glucose in patients after craniocerebral injury surgery. Methods:90 patients with brain injury after craniotomy were randomly divided into control group and propofol group. The β-EP value,HSP70 and lactic acid were tested at the moment the patient was transferred into intensive care unit(ICU)immediately and at 24,48 and 72 hours later,and the blood glucose was tested every 4 hours. Result:At 24,48 and 72 hours later,the β-EP,HSP70 and blood lactic acid levels of propofol group were significantly lower than that in control group,difference was statistically significant(P〈0. 05). The first 4 hours after transfer into ICU,the blood glucose levels of control group(11. 58 ±2. 65)mmol/L was higher than that of propofol group(9. 61±1. 71)mmol/L, difference was statistically significant( P=0 . 024 ). Conclusion:Propofol can reduce the levels of β-EP,HSP70,blood lactatic acid and blood glucose in the course of stress reaction after traumatic brain injury operation and enhance the adjustment of stress reaction of human body.
