摘 要:目的:观察放射性粒子植入联合支气管动脉灌注(BAI)化疗非小细胞肺癌(NSCLC)的临床疗效。方法:符合NSCLC患者88例,随机均分成观察组和对照组,观察组采用放射性粒子植入联合BAI化疗,对照组采用单一的BAI化疗;评价治疗后1月、3月及6月时的治疗效果,比较死亡率及恶心、腹泻、脱发及骨髓抑制等不良反应。结果:两组患者均完成4个周期的治疗,治疗后1、3、6个月时观察组有效率高于对照组,差异有统计学意义(P<0.05);观察组治疗后1月和3月无死亡病例、治疗6月死亡1例,死亡例数明显低于对照组,差异有统计学意义(P<0.05);观察组患者恶心、呕吐、脱发及骨髓抑制反应的发生率低于对照组(P<0.05)。结论:放射性粒子植入联合BAI化疗对NSCLC的疗效优于单一的BAI灌注化疗。Objective:To observe the clinical curative effect of radioactive seed implantation com-bined with bronchial artery infusion( BAI ) in treatment of non-small cell lung cancer( NSCLC ). Methods:88 cases of NSCLC patients were randomly divided into the observation group(44 cases) and the control group(44 cases). The observation group was treated with radioactive seed implantation combined with bronchial artery infusion while the control group treated with BAI chemotherapy. The curative effect was evaluated 1 month,2 months and 3 months after treatment. The mortality and nau-sea,diarrhea,hair loss and bone marrow suppression and other adverse reactions were compared be-tween the observation group and the control group. Results:The two group completed 4 cycles of treat-ment. After 1,3,6 months of treatment,the curative effect rate in the observation group was higher than that of the control group,and the differences were statistically significant(P〈0. 05);There were no deaths in the observation group after treatment in January and March,the treatment of 1 cases of death in June,the number of deaths was significantly lower than the control group,the difference was statistically significant( P 〈0 . 05 ). There were no deaths in the observation group 1 month and 3 months after treatment,and there was 1 case of death 6 months after treatment. The number of deaths in the observation group was significantly lower than that of the control group,and the difference was statistically significant(P〈0. 05). The incidence of nausea,vomiting,hair loss and bone marrow suppression in the observation group was significantly lower than their counterparts in the control group ( P〈0 . 05 ). Conclusion:The efficacy of radioactive seed implantation combined with BAI chemo-therapy in the treatment of NSCLC is better than that of single BAI perfusion chemotherapy.
