补阳还五汤对急性脑梗死患者血清白细胞介素-1β、白细胞介素-6、白细胞介素-18表达的影响

摘　　要：目的观察补阳还五汤对急性脑梗死患者血清白细胞介素(IL)-1β、IL-6、IL-18表达的影响。方法将90例急性脑梗死气虚血瘀证患者随机分为2组,对照组45例应用西医常规治疗,治疗组45例在对照组治疗基础上应用补阳还五汤治疗。2组疗程均为8周。观察2组临床疗效及中医证候疗效;比较2组治疗前后美国国立卫生研究院卒中量表(NIHSS)评分、血清IL-1β、IL-6及IL-18水平变化。结果总有效率治疗组81.40%,对照组61.36%,2组总有效率比较差异有统计学意义(P<0.05),治疗组临床疗效优于对照组。中医证候有效率治疗组76.74%,对照组56.82%,2组中医证候有效率比较差异有统计学意义(P<0.05),治疗组中医证候疗效优于对照组。2组治疗后NIHSS评分、IL-1β、IL-6及IL-18水平均较本组治疗前均降低(P<0.05),且治疗组降低较对照组明显(P<0.05)。结论益气活血中药补阳还五汤联合西医常规治疗急性脑梗死,可显著改善患者临床疗效、中医证候疗效,明显降低血清IL-1β、IL-6及IL-18水平,进而保护脑组织。Objective To observe the effects of Buyang - huanwu decoction on the expressions of serum in-terleukin - 1β（IL - 1β）,IL - 6 and IL - 18 in patients with acute cerebral infarction（ACI）. Methods 90 ACI patients with Qi deficiency and blood stasis type were randomly divided into two groups. The control group（n = 45） received western routine treatment,and the treatment group（n = 45）received Buyang - huanwu decoction on the basis of control group treatment,continuously treatment for 8 weeks. The clinical effects and the Chinese medicine symptom curative effects were observed in two groups. The National Institutes of Health Stroke Scale（NIHSS）,serum IL - 1β,IL - 6 and IL - 18 before and after treatment were compared between two groups. Results The total effective rate in treatment group（81. 40% ）was superior to that in control group（61. 36% ,P 〈 0. 05）. The Chinese medicine symptom curative rate in treatment group（76. 74% ）was superior to that in control group（56. 82% ,P 〈0. 05）. The score of NIHSS ,the serum IL - 1β,IL - 6 and IL - 18 after treatment were decreased in two groups（P〈 0. 05）,and the decrease in treatment group was more obvious than control group（ P 〈 0. 05 ）. Conclusion Buyang - huanwu decoction combined with western routine treatment can significantly improve the clinical effects and the Chinese medicine symptom curative effects on treatment of ACI,and decrease the serum IL - 1β,IL - 6 and IL -18,and then protect cerebral tissue.
