骨化三醇联合钙剂预防性服用对甲状腺癌全切术后低钙血症患者的临床疗效及血清PTH的影响

摘　　要：目的探究骨化三醇联合钙剂预防性服用对甲状腺癌全切术后低钙血症（Post thyroidectomy hypocalcemia,PHC）患者的临床疗效及血清PTH的影响。方法选取2014年10月-2015年12月温州医科大学附属二院收治的甲状腺癌全切术后患者68例,随机分为2组,每组34例。对照组在常规术后护理等基础上给予钙尔奇D片,观察组在对照组服药基础上给予口服骨化三醇,均连续给药7天,观察并记录服药后第1、2、3、5、7天2组患者血清钙、甲状旁腺激素（parathyroid hormone,PTH）水平及低钙血症发生情况。结果与对照组服药后同一时间点比较,观察组第3、5、7天血清钙、PTH水平均较高（P〈0.05）;观察组低钙血症及症状性低钙血症发生率较低（P〈0.05）;2组给药后均未见不良反应。结论骨化三醇联合钙剂预防性服用能有效促进甲状腺癌全切术后甲状旁腺功能的恢复,降低低钙血症的发生,且临床疗效较好,无不良反应发生。Objective To investigate the effect of hypocalcemia clinical curative by taking preventive calcitriol combined with calcium supplementsin postoperative patients with all cut thyroid cancer. Methods 68 patients from October 2014 to December 2015 in our hospital with thyroid cancer postoperative were collected and randomly divided into two groups,34 cases in each group. Patients in control group were treated with caltrate with vitamin D tables on the base of routine postoperative care,patients in experiment group were treated with oral of calcitrio on the base of control group,two groups were treated for 7 days,observed and recorded the serum calcium,parathyroid hormone（ PTH） levels and incidence of hypocalcemia after taking medicine 1,3,5,7 day. Results Compared with the control group after taking medicine the same time point,the experiment group serum calcium and PTH levels were higher at 3,5,7 day（ P〈0. 05）; compared with the control group,the experiment group hypocalcemia and symptomatic hypocalcemia incidence rate were lower（ P〈0. 05）,and there were no adverse reactions after taking medicine. Conclusion Calcitriol combine with calcium supplements in the treatment of thyroid cancer postoperative can promote thyroid function recovery,reduce the incidence of hypocalcemia,and clinical curative effect is better,have no obvious adverse effect.
