HPV联合TCT在宫颈癌前病变筛查中的临床价值分析

摘　　要：目的探讨联合人乳头瘤病毒（HPV）-DNA与宫颈液基薄层细胞学检测（TCT）在宫颈癌前病变筛查中应用的临床价值。方法选取2014年3月-2015年12月在第四军医大学西京医院妇产科进行宫颈癌前病变筛查的1670例患者,联合进行TCT和HPV-DNA检测及阴道镜下活检,以病理检测结果作为金标准,对比二者的一致性。结果 TCT、高危HPV-DNA及联合检测方法联合检测的阳性检出率分别是14.85%、15.03%、24.13%,联合检测优于单独检测,差异有统计学意义（P〈0.05）,TCT检测筛查宫颈癌前病变的敏感度、特异度及阴性预测值分别是89.05%、91.78%、98.94%,HPV-DNA筛查宫颈癌前病变的分别是91.24%、91.78%、98.95%,TCT联合HPV-DNA的筛查宫颈癌前病变分别是99.27%,82.58%,99.92%。结论 TCT联合HPV-DNA检测对于宫颈癌前病变筛查阳性检出率明显升高,并且具有较高的阴性预测值,对宫颈癌前病变的早期筛查具有较高的临床价值。Objective To explore the clinical value of liquid based cytology test（ TCT）-DNA combined with cervical liquid based cytology test（ HPV） in the diagnosis of cervical precancerous lesions. Methods From March 2014 to December 2015 in our hospital accepted 1670 cases cervical precancerous lesion screening patients,simultaneously TCT and HPV DNA testing and colposcopy biopsy,pathological examination results as diagnostic standard,and compared the consistency. Results The positive detection rate of TCT,and high-risk HPV-DNA and combination methods respectively was14. 85%,15. 03%,24. 13%,the combination methods was better than each single method,the difference was statistically significant（ P〈0. 05）. The sensitivity,specific degrees and negative predictive values of thinprep cytologic test in screening of cervical cancer precursor lesions were 89. 05%,91. 78%,98. 94%,HPV DNA screening for cervical precancerous lesions were 91. 24%,91. 78%,98. 95%,TCT and HPV DNA of cervical cancer screening before lesions were 99. 27%,82. 58%,99. 92%. Conclusion TCT combined with HPV-DNA detection was applied to the positive detection rate of cervical precancerous lesions,and has a high negative predictive value. It has a high clinical value for early screening of cervical precancerous lesions.
