联合检测外周血胃泌素17及胸苷激酶1水平对胃癌的临床诊断价值分析

摘　　要：目的探讨血清胃泌素17(G17)、胸苷激酶1(TK1)联合检测在胃癌中的临床诊断价值。方法选取徐州市肿瘤医院2016年1月-2016年5月收治的112例胃部疾病患者,按照其病变类型纳入胃癌组(n=46)、胃溃疡组(n=34)、慢性胃炎组(n=32),并选取同期28例健康体检者纳入健康组。分别采用ELISA法和化学发光结合斑点杂交技术检测受试者血清中G17、TK1的含量,并进行组间比较。结果胃癌组血清G17、TK1水平高于健康组、慢性胃炎组和胃溃疡组,差异有统计学意义(P<0.05)。与单独检测G17和TK1比较,联合检测G17和TK1的敏感性明显升高,差异有统计学意义(P<0.05),特异性无明显变化(P>0.05)。结论单项肿瘤标志物检测在胃癌的诊断中应用价值有限,G17和TK1联合检测有助于提高胃癌的诊断性能和早期检出率,有助于胃癌和其他胃部疾病的鉴别诊断。Objective To evaluate the clinical value of serum Gastrin-17 （G17） and Thymidine kinase-1 （TK1） combined detection in the diagnosis of gastric cancer. Methods One hundred and twelve patients were selected with gastric diseases in our hospital from Jan. 2016 to May. 2016. According to the pathological types, the subjects were di- vided into gastric cancer group （ n = 46） , gastric ulcer group （ n = 34） and chronic superficial gastritis （CSG） group （ n = 32）. Meanwhile, 28 healthy subjects were selected as the healthy group. The ELISA and Chemiluminescence combined with Dot Blot Hybridization were used to detect the serum levels of G17, TK1 in all the subjects. Results The levels of G17, TK1 in patients with gastric cancer were significantly higher than those in patients with gastric ulcer, chronic superficial gastritis and healthy subjects （P 〈 0.05）. The sensitivity predictive value of combined detection of G17 and TK1 was significantly higher than single detection. Conclusion The clinical value of single detection in the diagnosis of gastric cancer is limited. The combined detection of G17 and TK1 is beneficial to improve the diagnosis performance as well as to increase the diagnostic rate for gastric cancer.
