血栓弹力图与常规凝血试验对肝硬化患者凝血功能的比较分析

摘　　要：目的结合血栓弹力图(TEG)与常规凝血试验对肝硬化患者凝血功能检测进行评价。方法随机选择青岛市第六人民医院住院确诊的慢性乙型肝炎肝硬化患者30例为肝硬化组,选择同期健康体检者30名为对照组,两组入选者清晨空腹抽血,用常规凝血试验和TEG两种方法,检测两组人群凝血状况。结果常规凝血试验中,与对照组相比,肝硬化组PLT数量明显降低(P<0.01);肝硬化组PT、PT-INR、APTT、TT较对照组PT、PT-INR、APTT、TT均明显延长(P<0.01);肝硬化组FIB含量明显低于对照组(P<0.01);TEG检测表明,肝硬化组R和K时间较对照组明显延长(P<0.01);肝硬化组Angle、MA、CI均显著低于对照组(P<0.01)。结论常规凝血试验与TEG都能够检测出肝硬化患者凝血功能紊乱及出血倾向,两种方法结合使用,对肝硬化患者凝血功能检测更为准确。Objective To evaluate the value of the thrombelastography （TEG） and routine coagulation test for blood coagulation of liver cirrhosis patients. Methods Thirty patients with cirrhosis were randomly selected as the cirrhosis group and thirty healthy people during the same period were selected as the control group. Fasting venous blood was used in the test. TEG and routine coagulation test were used to analyze the function of blood coagulation. Results Routine coagulation tests showed that：the number of PLT in cirrhosis group was reduced significantly than that in control group （P 〈 0.01）. The PT, PT-INR, APTT, TT in cirrhosis group were extended significantly than those in control group （P 〈 0.01 ）. FIB content in cirrhosis group was reduced significantly than that in control group （P 〈 0.01 ）. TEG results showed that： R and K in cirrhosis group were extended significantly than those in control group （P 〈 0.01 ）. Angle, MA, and CI in cirrhosis group were decreased significantly than those in control group （P 〈 0.01 ）. Conclusion The routine coagulation and TEG both can detect the blood coagulation disorders in patients with cirrhosis. The combination of the two methods can improve the detection rate for blood coagulation of liver cirrhosis patients.
