不同铂类物质对晚期非小细胞肺癌患者的临床疗效观察

摘　　要：目的研究不同铂类物质联用吉西他滨后对晚期非小细胞肺癌患者的临床治疗效果和安全性。方法回顾性分析124例晚期非小细胞肺癌患者的临床相关资料,根据化疗方法的不同分为顺铂组（58例）和奥沙利铂组（66例）。两组肺癌患者分别采用吉西他滨联用顺铂和奥沙利铂联用吉西他滨方案化疗,对两组肺癌患者的临床疗效和不良反应发生情况等安全性指标进行比较。结果在临床疗效方面,两组并不存在显著性差异（P〉0.05）,两组中位无进展时间相比较也无显著性差异（P〉0.05）。与治疗前相比,两组体质量和KPS评分均有显著提高（P〈0.05）,且奥沙利铂组显著优于顺铂组（P〈0.05）。同时奥沙利铂组Ⅲ~Ⅳ级不良反应发生率显著低于顺铂组（P〈0.05）;除了肾功能异常不良反应外,奥沙利铂组患者Ⅱ~Ⅲ级不良反应发生率显著低于顺铂组,差异有统计学意义（P〈0.0 5）。结论吉西他滨联用奥沙利铂进行化疗,相较于顺铂临床疗效相当,但是其能显著提高非小细胞肺癌患者的生活质量和减少不良反应发生。Objective To investigate clinical effects and safety of cisplatin or oxaliplatin combined with gemcitabine chemotherapy in initial treatment of patients with advanced non-small cell lung cancer.Methods A retrospective analysis of clinical data of 124 elderly patients with advanced non-small cell lung cancer were conducted.According to the chemotherapy methods,they were divided into cisplatin group（ 58 cases） and oxaliplatin group（ 66 cases）.The 2 groups respectively received gemcitabine combined with cisplatin and gemcitabine combined with oxaliplatin,clinical curative effects and adverse reactions of the 2groups were compared.Results There was no significant difference in clinical efficacy and median progression free survival time between the 2 groups（ P 0.05）.In terms of safety,compared with before treatment,the body weight and KPS scores of the 2groups of patients with lung cancer were significantly increased（ P 0.05）,and oxaliplatin group was significantly superior to cisplatin group（ P 0.05）.At the same time,Ⅲ-Ⅳ grade adverse reactions of oxaliplatin group was significantly lower than that of cisplatin group（ P 0.05）.Conclusion Gemcitabine combined with oxaliplatin has similar efficacy with gemcitabine combined with cisplatin,but it can significantly improve the quality of life of patients with non-small cell lung cancer and reduce adverse reactions.
