腔镜辅助下保乳术对Ⅰ~Ⅱ期乳腺癌的有效性和安全性研究

摘　　要：目的探讨腔镜辅助下乳腺癌保乳术治疗Ⅰ~Ⅱ期乳腺癌的疗效及其不良反应。方法 84例乳腺癌患者,随机分为治疗组43例和对照组41例;两组术前均给予新辅助化疗方案(AC序贯T);对照组采取传统保乳术治疗。治疗组采取腔镜辅助下乳腺癌保乳术治疗。比较两组手术时间、平均引流量、拔管时间及生存质量。记录两组不良反应。结果治疗组在手术时间、平均引流量及拔管时间方面均明显优于对照组(P<0.01);术后6个月,治疗组患者的体型和未来看法评分显著高于对照组,性功能、性乐趣、脱发烦恼、乳房症状、手臂症状及系统疗法副作用评分均显著低于对照组(P<0.01)。两组患者不良反应多为Ⅰ~Ⅱ级,治疗组上肢水肿的发生率明显低于对照组(P<0.05)。结论腔镜辅助下乳腺癌保乳术治疗Ⅰ~Ⅱ期乳腺癌的疗效确切,且安全性好,值得临床应用。Objective To investigate the effectiveness and adverse reactions of breast conserving surgery by endoscope for stageⅠ ~ Ⅱ breast cancer.Methods 84 breast cancer patients were randomly divided into the treatment group（ 43 cases）and the control group（ 41 cases）.The 2 groups were performed preoperative neoadjuvant chemotherapy regimens.First,cyclophosphamide（ CTX） 75 mg / m^2 and adriamycin amycin（ Ad） 50 mg / m^2 were given for 3 weeks.Intravenous drip of Docetaxel（ DOC）60 mg / m^2 about 1 hour after one period of treatment of the CTX and Ad of 3 weeks.Above all 6 cycles periods of treatment with1 times / cycle were adopted.The control group was treated with traditional breast conserving surgery.The treatment group were additionally given breast conserving surgery by endoscope.Operation time,average flow,extubation time,and quality of life of the 2groups were compared.Adverse reactions of the 2 groups were recorded.Results Operation time,average flow and extubation time of the treatment group were evidently lower than those of the control group（ P 0.01）.After 6 months of operation,the size and the future view score of the treatment group was significantly higher than the control group（ P 0.01）.Sexual function,sexual pleasure troubles,hair loss,breast symptoms,arm symptoms and treatment side effects scores were significantly lower than those of the control group（ P 0.01）.Adverse reactions were Ⅰ ~ Ⅱ level.The incidence of upper limb edema of the treatment group was obviously less than that of the control group（ P 0.01）.Conclusion Breast conserving surgery by endoscope for stageⅠ ~ Ⅱbreast cancer has exact effect and good safety,and it is worthy of clinical application.
